摘  要:目的探讨11C-胆碱PET/CT显像在检测颅脑胶质瘤术后肿瘤残留或复发病灶的临床应用价值。方法20例颅脑胶质瘤术后疑为肿瘤复发或残留的患者均同时行11C-胆碱及18F-FDG颅脑显像,最后诊断结合手术后病理检查和临床随访。结果20例患者18F-FDG显像真阳性4例,真阴性3例,假阳性2例,假阴性11例,准确性35.0%(7/20例),敏感性26.6%(4/15例),特异性60%(3/5例),阳性预测值66.6%(4/6例),阴性预测值20%(3/15例);11C-胆碱显像,真阳性16例,真阴性3例,假阳性1例,假阴性0例,准确性95.0%(19/20例),敏感性100%(16/16例),特异性75%(3/4例),阳性预测值94.1%(16/17例),阴性预测值18.7%(3/16例)。结论11C-胆碱检测颅脑胶质瘤术后肿瘤残留或复发病灶价值优于18F-FDG显像。Objective To investigate the clinical value of nC-eholine PET/CT in detecting the remnant or relapse of cerebral gliomas after operation. Methods Taking ^18F-FDG and ^11C-choline PET/CT on 20 suspicions of the remnant or relapse tumor after operation in patients with cerebral gliomas. The results of PET/CT dignosis were compared with the pathology diagnosis after operation and clinical follow up. Results In 20 patients, the true positive of the diagnoses by ^18F-FDG PET/CT was 4 cases, the true negative 3 cases; the false positive 2 cases,the false negative 11 cases,the accuracy was 35% (7/20) ,the sensitivity was 26.6% (4/15) and the specificity was 60% (3/5), the positive predictive value was 66.6% (4/6)and the negative predictive value was 20% (3/15) ;the true positive of the diagnoses by ^18C-choline PET/CT was 16 cases, the true negative 3 cases, the false positive 1 case, no false negative case, the accuracy was 95 % ( 19/20), the sensitivity was 100% ( 16/16 ) and the specificity was 75 % ( 3/4 ), the positive predictive value was 94.1% ( 16/17 ) and the negative predictive value was 18.7% (3/16). Conclusion n C-choline PET/CT is superior to ^18F-FDG in detecteding the remnant or relapse tumors of cerebral gliomas after operation.
