摘 要:目的观察西咪替丁对体外循环心脏直视手术患者红细胞免疫功能的保护作用。方法随机选取2004年1月至2005年1月之间的30例风心病瓣膜替换患者作为观察对象,分为2组,实验组(西咪替丁组)和对照组,对两组患者术后外周血红细胞C3b受体花环率(RBC-C3bRR)、红细胞免疫复合物花环率(RBC-ICR)的动态变化进行对比研究。结果两组术后RBC-C3bRR、RBC-ICR均迅速下降,术后1、3、5d较术前明显降低(P〈0.01),实验组RBC-C3bRR于术后5、7、14d明显高于对照组(P〈0.01),于术后7d恢复至接近术前水平(P〉0.05),14d时较术前略高(P〈0.05)。对照组RBC-C3bRR于术后14d才恢复至术前水平(P〉0.05)。实验组RBC-ICR于术后5、7、14d明显高于对照组(P〈0.05),实验组、对照组均于术后14d恢复至术前水平(P〉0.05)。结论西咪替丁对体外循环心脏直视手术患者的红细胞免疫功能具有保护作用。Objective To explore the protective effects of cimetidine on the immune function of red blood cells in patients undergoing cardiopulmonary bypass. Methods From Janurary 2004 to Janurary 2005,30 patients with rheumatic heart diseases undergoing valve transplants were selected and randomly divided into two groups: experimental group (cimetidine group) and control group. The comparisons of dynamical changes of RBC-C3bRR and RBC-ICR between two groups were studied. Results The RBC-C3bRR and RBC-ICR in two groups were decreased rapidly after operation, markedly decreased on the postoperative day 1,3 and 5 as compared with those before operation (P 〈 0.01 ). In cimetidine group, RBC-C3bRR levels on the postoperative day 5,7 and 14 were significantly higher than in control group (P 〈 0.01 ) and approximately reached the preoperative level at 7th day ( P 〈 0.05 ), slightly higher than preoperative level at 14th day (P 〈 0.05 ). RBC-C3bRR levels in control group reached the preoperative level at 14th day after operation (P 〉0.05). RBC-ICR levels in cimetidine group were higher than in control group on the postoperative day 5,7 and 14 ( P 〈 0.01 ). RBC-ICR levels in two groups reached the preoperative levels at 14th day ( P 〉 0.05 ). Conclusion The cimetidine possessed the protective effects against the immune function of red blood cells in patients undergoing cardiopulmonary bypass.
