正常糖耐量胰岛β细胞功能及胰岛素敏感性的观察

摘　　要：[目的]研究正常糖耐量(NGT)胰岛素抵抗(IR)和胰岛β细胞功能变化。[方法]将121例NGT分为两组,A组76例餐后0.5h及或1h血糖(0.5h、1hPG)<10mmol/L,B组45例0.5h、1hPG>10mmol/L。糖耐量异常(IGT)62例为C组。测定口服糖耐量试验(OGTT)及胰岛素释放试验(OGIRT)。采用胰岛素敏感性指数(IAI)、HOMA法HOMA-IR指数评估胰岛素敏感性,HOMA-β、糖负荷后30min胰岛素增量与葡萄糖增量的比值(ΔI30/ΔG30)分别评估基础状态下及糖负荷后的早期胰岛β细胞分泌功能。[结果]3组间HOMA-β差异无统计学意义(P>0.05);B组、C组ΔI30/ΔG30、IAI低于A组;HOMA-IR高于A组,差异均有统计学意义(P<0.05,P<0.01)。[结论]与0.5h、1hPG<10mmol/L的NGT比较,0.5h、1hPG>10mmol/L的NGT存在明显的IR及胰岛素分泌缺陷。从NGT0.5h、1hPG正常到IGT,胰岛素抵抗有逐渐加重、胰岛素分泌逐渐恶化趋势。[Objective] To investigate the insulin resistance （IR） and function changes of pancreatic islet β-cell in normal glucose tolerance （NGT）. [ Methods ] 121 subjects of NGT were divided into two groups. The postprandial blood sugar of 76 subjects in group A were below 10mmol/L. The postprandial blood sugar of 45 subjects in group B were above 10mmol/L. 62 subjects with IGT were in group C. Oral glucose tolerance test （OGTF） and oral glucose insulin releasing test （OGIRT） were used. Insulin action index （IAI） and homeostasis model assessment （HOMA） were applied to assess the status of insulin resistance （HOMA-IR）. 30 minutes after the glucose load, HOMA-β and the ratio of the increment of insulin to that of plasma glucose （△I30/△G30） were used to evaluate the secretion function of β-cells of pancreatic islet under the basic states and after the glucose loads. [ Results ] There were not significant differences in HOMA-β among the three groups （P 〉 0.05）. △I30/ △G30 and IAI in group Band group Cwere both lower than that in group A. The HOMA-IR of subjects was higher in group B and group C than that in group A（P〈 0.05, P〈 0.01）. [Conclusion] Comparing with group A, the group B has obvious insulin resistance and β-cell deterioration. Reviewing 3 groups, the insulin resistabce levels rise continuously from group A, group B to group C, insulin secretion function go down gradually.
