摘 要:目的通过对感音神经性聋患儿的多频听觉稳态反应(multiple frequency auditory steady-state response,MFASSR)测试结果进行分析,并比较其在0.5kHz处与40Hz听相关电位(40Hz auditory event related potential,40Hz AERP)对客观听阈评估的准确性,为MFASSR临床应用提供指导。方法对感音神经性聋儿进行纯音测听、ABR、40Hz AERP和MFASSR测试。MFASSR与ABR、40Hz AERP测试均在睡眠状态下进行。按照测试结果分为ABR未引出组与ABR引出组。结果①MFASSR在0.5kHz处引出率比40Hz AERP低。②0.5kHz MFASSR反应阈对纯音听阈的评估较1、2、4kHz MFASSR反应阈对纯音听阈的评估差。③以纯音听阈为标准,在0.5kHz处MFASSR与40Hz AERP对纯音听阈的评估差别具有统计学意义(P=0.001),说明,在0.5kHz处MFASSR对纯音听阈评估的准确性不如40Hz AERP。结论MFASSR反应阈对0.5kHz处纯音听阈的预测需要结合40Hz AERP来判断。Objective This paper is to compare testing findings by multiple frequency auditory steady- state response (MFASSR) with that of 40 Hz auditory event related potential (40 Hz AERP) in the evaluation of sensorineural hearing loss in children. Methods Children with sensorineural hearing loss were tested with pure-tone audiometry, ABR, 40 Hz AERP and MFASSR. All the subjects were asleep during ABR, 40 Hz AERP and MFASSR tests. The response thresholds of MFASSR at each carrier frequency were compared to corresponding thresholds of pure-tone audiometry, ABR, and 40 Hz AERP. Results The positive incidence of MFASSR at 0.5 kHz was significantly lower than that of 40 Hz AERP. Significant differences (P=0.001) were noted among carrier frequencies in evaluating pure-tone thresholds, the most noticeable difference being found at 0.5 kHz. Significant differences were found between the response thresholds of MFASSR and 40 Hz AERP at 0.5 kHz (P=0.001). The accuracy of MFASSR was lower than that of 40 Hz AERP in estimating pure-tone thresholds at 0.5 kHz. Conclusion MFASSR can be used to estimate the frequency-specific pure-tone thresholds for children with sensorineural hearing loss and 40 Hz AERP should be used to confirm the thresholds at 0.5 kHz.
