摘 要:目的探讨借助螺旋CT三维重建技术,通过测量阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hypopnea syndrome,OSAHS)患者清醒状态下咽腔容积在平静呼吸和müller呼吸时的变化来评价OSAHS患者咽腔形态和功能变化的可行性和有效性。方法11例OSAHS患者,分别于平静呼吸和müller呼吸时行螺旋CT扫描,范围从硬腭上缘至舌骨上缘,通过CT工作站进行三维重建成像,借助CT三维重建工作站自带软件分别测量两种状态下硬腭下缘至悬雍垂尖(腭咽腔)以及悬雍垂尖至会厌尖(舌咽腔)的咽腔容积。观察咽腔各部分容积的变化。结果在平静呼吸和müller呼吸时两部分咽腔容积大小有显著性差异,P<0.01。两种状态下大多数(7/11)腭咽腔容积变化率大于舌咽腔容积变化率,但差异无显著性。结论CT三维重建咽腔容积测量对于评价OSAHS患者的咽腔形态和功能具有较好的应用价值。OBJECTIVE To evaluate a measuring method of pharyngeal volume at normal breath and müller maneuver state using spiral computed tomography three-dimensional reconstruction technique in waking condition, and to study the pharyngeal morphology and function changes in obstructive sleep apnea hypopnea syndrome patients. METHODS Eleven patients with obstructive sleep apnea hypopnea syndrome (OSAHS) were taken CT scan in normal breath and müller maneuver state. The CT extent was from the upper edge of hard palate to the upper edge of hyoid. Then the pharynx image was reconstructed using the three-dimensional reconstruction software and the volumes of velopharynx and glossopharyngeum were measured at the two different stages. RESULTS There was a significant difference in volume between the eupneic and müller maneuver state (p 〈0.01). The volume change ratio of seven patients was larger than that of the others although there was no significant difference. CONCLUSION The measurement of pharyngeal volume using computed tomography three-dimensional reconstruction technique is good method to evaluate the pharyngeal morphology and function.
