荆州地区不同文化程度糖尿病患者疾病控制状况调查

摘　　要：目的了解荆州地区不同文化程度糖尿病患者疾病控制现状,为不同文化程度糖尿病患者的早期干预及治疗、护理决策提供依据。方法选择荆州地区病程1年以上的糖尿病患者600例,按文化程度分为"初中及以下"和"高中及以上"两组各300例。测量两组患者空腹血糖(FPG)、早餐后2 h血糖(P2hPG)、糖化血红蛋白(HbA1c)、尿微量白蛋白(UAb)及眼底情况,并进行比较;使用糖尿病控制状况量表(CSSD-70)进行测评,比较不同文化程度患者的疾病控制情况差异。结果初中及以下文化程度糖尿病患者FPG、P2hPG、HbA1c高于高中及以上文化程度患者(P<0.05);初中及以下文化程度糖尿病患者CSSD-70量表总分及生活习惯、生存技能、治疗目标、疾病知识维度得分均低于高中及以上文化程度患者(P<0.05)。结论初中及以下文化程度的糖尿病患者疾病控制状况差于高中及以上文化程度的患者,因此要加强文化程度较低的糖尿病患者的治疗和管理水平,加强糖尿病健康教育,大力推广新的治疗策略,预防晚期并发症,提高生活质量,降低医疗费用。Objective To explore the control of disease conditions of diabetic patients at different educational levels in Jingzhou to provide the guidelines for early diabetic intervention and treatment and provide proper nursing care for them.Methods 600 diabetic patients with a course of more than one year were selected as two groups according to their education backgrounds： 300 with junior middle（or lower） schooling and 300 with higher middle（or higher） schooling.FPG,P2hPG,HbA1c,UAb and eye ground were observed and compared.CSSD-70 was used to investigate the disease control conditions among them and the control conditions between the two groups were compared.Results The FPG,P2hPG,HbA1c from those with junior middle（or lower） schooling were higher than those with higher middle（or higher） schooling（P〈0.05）.The total score by CSSD-70 and the scores at life habit,living skills,treatment aim,knowledge about diabetic among those with junior middle（or lower） schooling were lower than those with higher middle（or higher） schooling（P〈0.05）.Conclusion The treatment and management for the diabetic patients with lower schooling should be enhanced.Health education about diabetic should be enhanced and new therapy should be spread to prevent late complications,increase of life quality and lower medical expenses.
