摘  要:目的评估急性颅脑损伤患者血浆部分凝血指标变化对判断损伤程度及预后的价值。方法检测71例急性颅脑损伤患者伤后24h、3d、7d、14d的D-二聚体(DD)、纤维蛋白原(Fg)血浆含量,结合临床格拉斯哥评分(GCS)分型及格拉斯哥预后评分(GOS)预后分级进行统计学分析。结果伤后24h内两项纤溶指标的检测结果与伤后7d、14d比较均有非常显著的差异,Fg检测结果与伤后3d比较差异也有统计学意义。DD含量与GOS呈负相关;Fg与GOS呈正相关。结论DD、Fg对判断颅脑损伤程度及预后有重要参考价值,可作为临床应用的监测指标。Objective To explore the correlation of plasma D -dimer and fibrinogen level with the severity and prognosis of head injury. Methods The plasma levels of D - dimer and fibrinogen were dynamically measured with immunoturbidimetry in 71 patients with head injuries and 71 normal controls. And the relationship between Glasgow coma score (GCS) , Glasgow outcome score (GOS) and the concentration of D - dimer and fibrinogen were also compared. Results The D - dimer and fibrinogen at 24h after injury were significantly different from patients in 7d and 14d, the fibrinogen at 24h after injury were different from patients in 3d . The concentration of D - dimer correlated negatively with GOS. The concentration of D - dimer correlated negatively with GOS. The concentration of fibrinogen correlated positively with GOS. Conclusions Dynamic detection of D - dimer and fibrinogen in plasma might be useful to judging the severity and prognosis of head injury at early stage.
