摘　　要：目的探讨2型糖尿病家系正常血糖一级亲属(FDR)腰围、脂联素、肿瘤坏死因子-α和白介素-6与胰岛素抵抗之间的关系。方法以FDR(50名)为观察组,30名年龄、性别相匹配的无糖尿病家族史的正常人为对照组,检测按不同腰围分组的Adiponectin、TNF-α和IL-6水平,同时测定空腹胰岛素(FINS)、总胆固醇(TCH)、三酰甘油(TG)和低密度脂蛋白胆固醇(LDL-C);用稳态模型法(HOMA)计算胰岛素抵抗指数(IRI),并对它们进行相关分析和逐步回归分析。结果FDR组按不同腰围亚组与正常对照组比较,IRI、TNF-α、脂联素3组间比较差异有显著性(P<0.01),IRI和TNF-α随腰围增加而升高,脂联素水平随腰围增加而下降;FDR组IL-6水平均高于正常对照组(P<0.05);腰围、脂联素是影响FDR者IRI的重要危险因素。结论FDR者存在胰岛素抵抗,抵抗程度主要受腰围和脂联素影响。[Objective] To study insulin resistance in the normoglycemic first-degree relatives of type 2 diabetics （FDR） and its relationship with waist circulation （WC）, adiponeetin, tumor necrosis faetor-α and interleukin-6. [Methods] A total of 50 euglyeemia FDR and 30 gender, age matched control subjects without family history of di abetes were involved. Fasting insulin blood lipids, adiponeetin, TNF-α and IL-6 were measured in all the cases, insulin resistance （IRI） was calculated by homeostasis model assessment, the relationship between them was analyzed. [Results] IRI and TNF-α of the sub-groups in FDR were significantly higher than these of controls. Serum adiponeetin of the sub-groups in FDR was significantly lower than that of controls. With the increase of WC, IRI and TNF-α were increased, and adiponeetin was decreased. Serum IL-6 in obesity was higher than controls; IRI was significantly correlated with WC and adiponectin. [Conclusion ] Insulin resistance is found in FDR which degree is influenced by waist circulation and adiponectin.
