更昔洛韦联合丙种球蛋白治疗先天性巨细胞病毒感染

摘　　要：目的:观察更昔洛韦联合丙种球蛋白治疗先天性巨细胞病毒(CMV)感染的疗效。方法:将60例CMV感染患儿随机分成两组。治疗组(n=32例)在常规保肝治疗基础上加用更昔洛韦5mg/kg静脉滴注,2次/d,连用14d,维持量5mg/kg,1次/d,连用14d。丙种球蛋白400mg/kg,1次/d,连用5d。对照组(n=28例)予常规保肝治疗。结果:治疗组血清总胆红素、谷丙转氨酶、CMV-DNA转阴率均明显优于对照组。治疗组总有效率为81.3%,无效率为18.7%。对照组总有效率57%,无效率为43%。结论:更昔洛韦联合丙种球蛋白治疗先天性巨细胞病毒感染疗效较好,值得临床推广。objective To investigate the therapeutic effects of Ganciclovir and IVIG on congenital cytomegalovirus（cmv）infections. Method 60 cases of congenital CMV infections were divided into the experimental group （ n = 32 ） with ganciclovir and IVIG and the control group with general treatment （ n =28）. The patients of the experimental group was treated with Ganciclovir 5mg/kg, intravenous drip every 12 hours, continous using for 14 days, then maintenance treatment with 5mg/kg,intravenous drip every day, continous 14 days. IVIG 400mg/kg, intravenous drip every day, continous 5 days. The patients of the control group was treated with routine method, then two groups were compared with clinical situation, laboratory examination and virology result. Results The changes of TBIL and ALT and negative rate in the experiental group were superior to that in the control group （ P 〈 0.01 ）. The efficient of the experimental group was 81.3% , and the control group was 57%. There was a significant difference between the two groups （ P 〈0.01 ）. Conclusion It is safe and effective to treat congenital CMV infections with Ganciclovir and IVIG,so can be widespread in clinical practice.
