扶正解毒颗粒与西药结合治疗60例长期接触镍的冠心病患者的临床研究

摘　　要：目的：研究中药扶正解毒颗粒（红芪、茯苓、当归、连翘,莪术、丹参等）对长期接触镍的冠心病患者的临床疗效及其对血清肌酸激酶（CK）、乳酸脱氢酶（LDH）、红细胞膜Na^＋-K^＋-ATP酶、Ca^2＋-ATP酶、一氧化氮（N0）、一氧化氮合酶（NOS）、Ni水平的影响。方法：将60例患者随机分为两组：A组30例,采用中药扶正解毒颗粒及西药倍他乐克和消心痛治疗;B组30例,仅用西药治疗。疗程均为4周。C组15例,为同地区非接触镍健康人群对照组。观察两组治疗前后临床症状、心电图的变化,检测肌酸激酶（CK）、乳酸（LD）、乳酸脱氢酶（LDH）、红细胞膜Na^＋-K^＋-ATP酶、Ca^2＋-ATP酶、血清一氧化氮（NO）、一氧化氮合酶（NOS）水平的变化及血清镍的改善。结果：与B组比较,A组临床症状,心肌酶CK、LDH、红细胞膜Na^＋-K^＋-ATP酶、Ca^2＋-ATP酶、一氧化氮（N0）、一氧化氮合酶（NOS）及血清镍的改善均优于B组（P〈0.05～0.01）。结论：扶正解毒颗粒降低血清镍,恢复心肌酶的活性,从而改善冠心病患者临床症状的异常。AIM： To evaluate the clinical effect of Fuzheng Jiedu Granule （Radix Hedysari, Poria, Radix Angelicae sinensis, Fructus Forsythiae, Rhizoma Curcumae, Radix et Rhlzoma Salviae miltiorrhizae, etc. ） on patients with coronary heart disease who have long-term contact with nickel, and observe its influence on the level of Creatine kinase （CK）, Lactate dehydrogenase （LDH） , Na^＋-K^＋ -ATPase, Ca^2＋ -ATPase in RBC membrane, Nitric oxide （NO） ,Nitric oxide synthase （NOS）,Nickel （Ni） in serum. METHODS： 60 cases were divided into two groups A and B randomly. 30 cases in group A were treated with Fuzheng Jiedu Granule combined with western medicine Betaloc and Isosorbide Dinirrate ;30 cases in group B were given only western medicine. The period of treatment was 4 weeks. Another 15 healthy cases selected as group C （healthy control group）. Before and after treatment, the clinical effects were observed, meanwhile, some biochemical indexes such as CK, LDH, Na^＋ -K^＋ -ATPase, Ca^2＋ -ATPase in RBC membrane, NO, NOS, Ni in serum were measured with clinic test methods. Finally, the experimental results were compared and analyzed in the three groups mentioned above. RESULTS： The therapeutic effect of symptoms and signs of group A was superior to that of group B （P 〈 0.05 ） ; compared with group C before treatment, there was a significant increase in the levels of serum CK, LDH, Ni and a significant decrease in the levels of serum NO, NOS, Na^ ＋ -K^＋ -ATPase, Ca^2＋ -ATPase in RBC membrane ; compared with group B after treatment ,the levels of serum CK, LDH.Ni in group A decreased （ P 〈 0. 05 - 0. 01 ） significantly ; Meanwhile, there was a remarkable increase in the activites of serum NO, NOS, Na^＋ -K^＋ -ATPase, Ca^2＋ -ATPase in group A （P 〈 0.05 - 0.01 ）. CONCLUSION： Fuzheng Jiedu Granule can decrease the serum Nickel, restore the activity of cardiac enzyme, and improve patients＇ symptoms.
