用重建心阻抗图观察冠心病患者心功能的血流动力学变化

摘　　要：目的用重建心阻抗图观察冠心病患者心功能的改变。方法实验分患者组和健康对照组,用重建非线性公式分别计算并统计两组的心搏出量(SV)、每分心排血量(CO)、心排血指数(CI)、左室心功指数(CWI)、左室射血分数(LVEF)、左室心收缩指数(LHI)、总外周阻力(TPR)、主动脉顺应性(AC)、左室舒张指数(LDI)、左室舒张末压(LVEDP)。结果重建心阻抗法监测的心功能不全组中TPR、LDI、LVEDP均明显高于对照组,而CWI两者则无明显差异(P>0.5),其余指标明显降低。结论上述结果符合患者及健康人的病理生理变化及血流动力学特点,重建心阻抗图应用于冠心病患者时应结合临床,能提供一定的参考价值。Objective To explore coronary heart disease patient＇s cardiac function change by reconstructed impedance cardiogram. Methods Reconstructed impedance differential cardiogram was performed in two adults groups （ patient group ,control group） and the cardiac function was calculated by reconstructed impedance cardiographic non-linear formula.Separately we record the two groups of cardiac function which are stroke volume （SV）,cardiac output （CO）,cardiac index （CI）,cardiac work index （CWI）,left ventricular ejection fraction （LVEF）,left heather index （LHI）,total peripheral resistance （ TPR）,aortic compliance （AC ）,left ventricle diastolic index （LDI）and left ventricular end-diastolic pressure （LVEDP）and to make statistical analysis.Results There hasn＇t manifest disparity in CWI between two groups （P〉0.5）,But LVEDP from patient group is obviously larger than from control＇s as well as TPR and LDI,while the others from patient group is smaller than from control＇s.Conclusion The date in experiment reflect the physiological and hemodynamic change of coronary heart disease patients and heahhy.When reconstructed impedance cardiogram monitoring is applied by coronary heart disease patients, physician should integrate clinic and make correct diagnosis and management.
