检测端粒酶和DNA异倍体诊断恶性腹水

摘　　要：目的:评价应用端粒酶和DNA异倍体指标诊断恶性腹水的价值。方法:良、恶性腹水标本各57份,应用端粒重复序列扩增-杂交-酶联免疫分析技术检测端粒酶、流式细胞术检测DNA异倍体,并与CEA相比较。结果:恶性腹水端粒酶、DAN异倍体、CEA的阳性率均明显高于良性腹水(P<0.01)。端粒酶、DNA异倍体及CEA指标诊断恶性腹水的特异性、敏感性、阳性预测值、阴性预测值、准确性分别为:93%、94.7%与89.5%;84.2%、73.7%与57.9%;92.3%、93.3%与84.6%;80.3%、75%与63%;88.6%、84.2%与73.7%。结论:应用端粒酶和DNA异倍体诊断恶性腹水具有较高的价值,其诊断价值高于CEA。Objective：To evaluate the value of detecting telomerase and DNA aneuploid in diagnosis of malignant ascites.Methods：57 cases of nonmalignant and malignant ascites were detected in this study.TRAP- Hyb-ELISA,Flow cytometry and magnetic antibody immunoenzymetric assay were respectively used to detect telomerase,DNA aneuploid and CEA.Results：The positive rate of telomerase,DNA aneuploid and CEA in malignant ascites were all obviously higher than nonmalignant （P 〈 0.01）.The specificities,sensitivities,positive predictive values,negative predictive values and accuracies of the three indexes were telomerase：93%,84.2%,92. 3%, 80.3% and 88.6%;DNA aneuploid＇94.7%,73.7%,93.3%,75% and 84.2%;CEA：89.5%,57.9%,84.6%,63% and 73.7%.Conclusions：Telomerase,DNA aneuploid and CEA are useful index for diagnosing of malignant ascites and the value of telomerase and DNA aneuploid are higher than CEA.
