职业紧张对火车司机血清中皮质醇浓度的影响

摘　　要：目的探讨职业紧张对火车司机血清中皮质醇浓度的影响。方法采用随机抽样方法抽取某铁路局的火车司机208人为调查对象,使用职业紧张因素量表（OSI）中的职业紧张来源部分对其职业紧张状况进行调查,采用放射免疫法分析血清中皮质醇浓度。结果火车司机血清中皮质醇的浓度为（447.48±148.72）ng/ml,95%的可信区间为427.15-467.81ng/ml。单因素分析发现,客车司机与货车司机血清中皮质醇浓度比较差异具有统计学意义（P〈0.05）;以每次连续乘务时间分组,组间血清中皮质醇浓度比较差异具有统计学意义（P〈0.05）;以年龄、现岗位工龄和文化程度等因素分组,各组间血清中皮质醇浓度比较差异无统计学意义（P〉0.05）。以年龄、工龄等非职业紧张因素和构成职业紧张因素的6个因子得分共计17个因素为自变量,以血清中皮质醇浓度为应变量进行多因素逐步回归分析（α入=0.05,α出=0.10）得知,影响血清中皮质醇浓度的主要因素是来源于经历与成就的职业紧张因素、岗位变量以及吸烟量。结论职业紧张对火车司机皮质醇分泌可能有影响,货车司机血清中皮质醇浓度高于客车司机。Objective To explore the effects of occupational stress on serum cortisol level in engine drivers.Methods 208 engine drivers in a certain railway bureau were investigated using occupational stress questionaires（OSI）.Meanwhile,blood samples were taken in morning for cortisol level detection by radio-immune-assay.Results The serum cortisol level of engine drivers was（447.48±148.72）ng/ml,and the 95% confidence interval（CI）was 427.15～467.81ng/ml,the freight engine drivers had higher level of serum cortisol than that of passenger engine drivers（P〈0.05）,and those with longer working time also had higher serum cortisol level（P〈0.05）,but which didn＇t related to age,present post-service length and education,etc..Stepwise regression analysis（αin=0.05,αout=0.10）showed that the primary impact factors on serum cortisol level were occupational stresses from experience ＆ achievement,working post and smoking.Conclusion Occupational stress might have some effect on serum cortisol level of engine drivers,and the level of serum cortisol in freight engine drivers was higher than that of passenger train drivers.
