微波热凝对肥厚性咽炎咽部病损组织PCNA表达的影响

摘　　要：目的观察微波热凝对慢性肥厚性咽炎局部病损组织增殖细胞核抗原(PCNA)表达的影响。方法诊断明确的慢性肥厚性咽炎患者70例,随机分为两组。微波热凝组35例(治疗组)采用微波热凝治疗,药物治疗组35例(对照组)采用常规药物治疗。两组患者均在治疗前和治疗2周后取咽后壁淋巴滤泡明显处同一部位病损组织,应用免疫组化方法进行PCNA检测,应用Ridit分析比较两组的结果差异。结果疗程结束后,两组的临床疗效差异具有统计学意义(P<0.05)。同时,PCNA表达的检测结果显示,不仅疗后两组的PCNA指数均明显低于治疗前(P<0.001),而且治疗组的治疗后PCNA指数又明显低于对照组(P<0.001)。结论微波热凝可能通过抑制咽部病损组织的细胞增殖活性而对慢性肥厚性咽炎发挥治疗作用。Objective To investigate the effect of microwave coagulation therapy in the expression of Proliferating cell nuclear antigen（PCNA） in the local mucosal tissue based on a study in a group of Patients with chronic hypertrophic Pharyngitis. Methods Seventy cases with definitely diagnosed chronic hypertrophic Pharyngitis were randomly divided into two groups with 35 in treating group （TG） treated by microwave coagulation therapy （MWCT） and 35 in controlling one （CG） treated by routinely used drugs in clinic Practice at Present Before and after the therapy, tissue samples were taken from the local focus with the most significantly hypertrophiclymphoid follicles at the same location respectively for the expressive activity determination of PCNA by means of immunohistochemical Procedures, with a Ridit analysis made to compare the differences of the indicators between the 2 groups following the therapy. Results By the end of therapy, it was shown that the signifcantly superior therapeutic effect of short term could be seen in TG to that Of CG （P〈0.05） Meanwhile not only there were significantly lowered indicators in the expressive activity of PCNA when compared that before treatment with that after treatment for these two groups respectively （P〈0.001） but also there was a significant difference Present in this indicator between the two groups after the therapy much more markedly inhibitory effect of microwave coagulation therapy shown on PCNA （P〈0001） .Conclusion Microwave coagulation therapy may gave rise therapeutic effect on chonic hypertrophic Pharyngitis through inhibiting cell Proliferating activity of the local focus mucosa in the Pharynx.
