摘　　要：目的探讨经阴道三维彩超在评价子宫动脉栓塞术治疗子宫肌瘤疗效中的应用价值。方法应用经阴道三维彩色多普勒超声对47例患者在子宫动脉栓塞术前后进行肌瘤检测,重点观察手术前后肌瘤体积、内部回声及血流情况的变化。结果①术后肌瘤内血流信号明显减少或消失,并且随着观察时间的不同,肌瘤内部可见多种回声改变;②子宫动脉栓塞术近期效果明显,术后1-6个月肌瘤体积逐渐缩小;③术后肌瘤体积的缩小程度与肌瘤内部回声、肌瘤位置、肌瘤内血流情况有关。结论经阴道三维彩色多普勒超声所获图像清晰、立体感强、测量瘤体体积准确,并可以显示肌瘤内外的立体血流分布情况,弥补了二维超声的不足,是评价子宫动脉栓塞术治疗效果的有效手段,对疗效的长期观察具有重要价值。Objective To discuss the application value of tmnsvaginal 3D color Doppler ultrasonography in evaluating effects of uterine artery embolization （UAE）. Methods Forty - seven cases of uterine myomas were investigated by transvaginal 3D color Doppler ultrsonography before and after UAE, and to study the change of volume, echo, and blood flow distribution of myoma. Results （1）Blood flow signal in myoma decreased significantly or disappeared after UAE. Myomas show various ultrasonographic appearance in different period after UAE. （2）UAE has significant effect in near future. In the period from 1 months to 6 months after operation, myoma volume kept on decreasing progressively. （3）The decrease in myoma volume was correlated with the location, echo, and blood flow of myoma. Conclusions Transvaginal 3D color Doppler ultrasonography can provide clear and stereoscopic images, avoid various disturbances, and directly show the myoma and supplying vessels. It is a new noninvasive technique and a good compensation for two dimensional ultrasound. It can be used for detecting the curative effect after UAE.
