巴曲亭——蛇毒凝血酶在阴道分娩中的止血作用

摘　　要：目的：观察巴曲亭——注射用蛇毒凝血酶在阴道分娩中止血的效果。方法：选取自2006年2～5月在该科正常阴道分娩的产妇80例，随机分为两组，实验组产妇分别于胎头拨露阴道口2～3cm时静脉滴注巴曲亭1kU并在会阴侧切后伤口局部湿敷稀释的1kU巴曲亭，胎儿娩出后立即肌肉注射催产素10U，同时静脉滴注催产素20u，对照组只用催产素，观察两组产妇会阴侧切伤口、产后2h和24h的出血量以及给药前、给药后1天肌体凝血功能的变化。结果：实验组产妇会阴侧切伤口及产后2h、24h出血量与对照组比较明显减少（P〈0．01）；同时手术前后凝血活酶时间（PT）、部分凝血活酶时间（APTT）、凝血活酶生成时间（TT）、纤维蛋白定量（FIB）无明显变化，同对照组比较无显著性差异（P〉0．05）。实验组病人应用巴曲亭后均未出现不良反应。结论：巴曲亭——注射用蛇毒凝血酶能减少阴道分娩中的出血，疗效明显，安全性好。Objective： To observe the hemostatic effect of intravenous injection Baquting in vaginal delivery. Methods： From February to May 2005, 80 deliveries in this department receiving vaginal delivery were divided into trial and control groups randomly. The puerperants in trial group were administered with intravenous drip infusion of 1 kU Baquting when head visible on vulval gapping 2 -3 cm while focal hydropathic compress of diluted 1 kU Baquting after lateral perineum excision, then were given intramuscular injection of 10 U oxytocin and intravenous drip infusion of 20 U oxytocin. The puerperants in control group were given with oxytocin. The homorrhagic volume of vulval wound and postpartum blood lose within 2 and 24 h were observed, blood coagulation function before and l d after operation were detected. Results： Volume of blood loss in trial group were obviously lower than those of control group （P 〈 0. 01 ） . Prothrombin time （PT）, thrombin time （TT）, activated partial thromboplastin time （APTY） and fibrin （FIB） before and 1 d after operation were of no difference （P 〉 0. 05 ） and no significant difference between two groups （ P 〉 0. 05 ） . There were no adverse drug reaction in trial group. Conclusion： Baquting could reduce haemorrhage in vaginal delivery, which is safe and effective.
