摘 要:目的:通过13城市新生儿CH筛查情况,总结中国主要城市和地区先天性甲状腺功能减低症(Congenital Hypo-thyroidism,CH)发病率,寻找规律,探讨对策。方法:筛查对象为出生72h后的新生儿,采足跟末梢血制成干血滤纸片,用FEIA法、EIA法、ELISA法或DELFIA法检测干血片中TSH含量,将初筛阳性儿召回确诊。结果:13城市共对3351818例新生儿进行了CH筛查,确诊CH患儿975例,CH发病率为0.291‰,总体来看,中国北部和南部地区CH发病率低于东部、中部和西部地区。讨论:CH高发病率地区和城市要认真研究本地区CH发病的主要原因,有针对性地采取预防措施,降低CH发病率,同时,对初筛阳性儿,在诊断时要注意高TSH血症儿的误诊问题。CH低发病率地区和城市要审视标本采集、递送、实验室质量和诊断等各环节,以避免漏检和漏诊。Objective: By screening congenital hypothyroidism (CH) data of 13 cities newborn baby to summarize the main city and area CH morbidity rates and analyze the difference causes and countermeasures. Methods: A drop of plantar blood was obtained from newborns 72 hours after delivery to dry onto a filter paper to test blood levels of TSH with FEIA, EIA, ELISA or DELFIA method. Screened positive cases were called back for futher diagnosis. Results: Among the screened 3 351 818 cases in 13 cities, 975 cases were diagnosed as CH, CH morbidity rate was 0. 291%o. Overall CH morbidity rate in northern and southern of China was lower than that in eastern, middle and western area. Conclusion: The higher CH morbidity rate areas and the cities should study the main reasons which cause high local CH morbidity rate and take preventive measures to reduce CH morbidity rate. However, the screen positive cases with higher TSH blood level must be paid attention to and be surely not to be misdiagnosed. The lower morbidity rate areas and the cities should carefully examine every chain such as specimen gathering, delivering, the lab quality and diagnosis and so on, aiming to avoid the occurrence of leaking examines and leaking diagnosis.
