Tei指数监测蒽环类药物对肿瘤患者右心功能的影响

摘　　要：目的探讨心肌做功指数(MPI,又称Teiindex)在评价蒽环类药物(ATC)对肿瘤患者右心功能早期损害方面的临床应用价值。方法测定并比较50例肿瘤患者ATC化疗前和化疗后6个月常规超声心动图参数及右室Tei指数参数。结果Tei指数各相关参数显示ATC化疗后右心室等容舒张时间IRT′较化疗前明显延长(P<0.05);右心室等容收缩时间(ICT′),右室射血时间(ET′)较化疗前无明显差异(P>0.05),但右心室Tei指数较化疗前有显著性差异(P<0.05)。左室收缩功能指标射血分数(EF)化疗前后无显著性差异(P>0.05),仅4例(8%)出现右心室舒张功能指标异常。结论常规超声心动图可用于评价放疗对右心功能的影响,与常规超声相比,Tei指数能更早、更敏感地评价ATC对肿瘤患者右心功能损害。Objective To explore the clinical value of Tei index as assessing right ventricle function damage in malignant patients received anthracycline chemotherapy. Methods 50 patients were examined by echocardiography and Tei index before anthracycline chemotherapy and six months later after anthracycline chemotherapy. Results Parameters of Tei index showed that right ventricular isovolumic relaxation time （IRT′）were significantly prolonger （P〈0.05）, isovolumic systolic time （ICT′） and ejection time （ET′） had no statistic significance （P〉0.05）, but right ventricular Tei index was significantly higher（P〈0.05） compared with those data before chemotherapy. Parameters acquired from common echocardiography showed that abnormalities of routine diastolic function index of right ventricle appeared in 4 patients （8%）, while left ventricular systolic function （EF） had no statistic significance compared with those data acquired pre-chemotherapy （P〉0.05）. Conclusion Tei index are more sensitive in detecting abnormalities of right ventricle function.
