摘 要:目的探讨2型糖尿病(diabetes mellitus,DM)及其相关因素对老年患者认知功能的影响。方法对老年单纯2型糖尿病组(单纯DM组)38例、2型糖尿病合并缺血性脑损害组(DM并脑缺血组)34例及正常对照组31例行听觉事件相关电位P300及简易智能量表(MMSE)检测。结果MMSE评分单纯DM组与正常对照组比较差异无统计学意义(P>0.05);DM并脑缺血组较正常对照组降低(P<0.01)。老年2型糖尿病各组P300的N2、P3潜伏期(Lat)较正常对照组延长(P<0.01),P3波幅(Amp)较对照组降低(P<0.01);DM并脑缺血组N2、P3潜伏期较单纯DM组延长(P<0.01),波幅无明显差异(P>0.05)。多元线性回归分析显示,老年2型糖尿病患者P3潜伏期与患者年龄、病程、糖化血红蛋白(GHB)及合并缺血性脑损害正相关,P3波幅与患者年龄、病程、GHB负相关,N2潜伏期与患者年龄、病程、合并缺血性脑损害正相关。结论2型糖尿病可引起老年患者认知功能损害,其中糖尿病病程、GHB及合并缺血性脑损害是认知功能损害的主要影响因素,P300检测可作为老年糖尿病认知损害早期诊断和动态监测的敏感指标。Objective To determine the effect of type 2 diabetes and related factors on the cognitive function in older persons. Methods 72 patients with type 2 diabetes mellitus(DM) aged 60 to 84 years,and 31 normal controls at the same age range participated in this study,and were divided into DM with cerebral ischemia group and DM without cerebral ischemia group proved by CT scanning. Audio event-related potentials and mini-mental state examination(MMSE) were performed in all the groups ,and the results were analyzed statistically. The relation of ERP parameter with glyeosylated hemoglobin (GHB), age, sex, educational level, course of disease and ischemic brain damage were analyzed. Results No significant difference in the MMSE scores was found between the DM without cerebral ischemia group and the normal control group(P〉 O.05), but found between the DM with cerebral ischemia group and the normal control group(P〈0. 01). The latency of N2 and P3 were prolonged and the amplitude decreased significantly in the DM without cerebral ischemia group and the DM with cerebral ischemia group as compared with the normal controls respectively. The latency of N2 and P3 were prolonged in the cerebral ischemic group was also prolonged significantly than that in the non- cerebral ischemia group(P〈0.01) ,but no significant difference was found between these two groups in the amplitude. In multiple linear regression model (P〉O.05), there was positive relation between the latency of P3 and GHB, age, course of disease and ischemic brain damage respectively in the DM group; there was negative relation between the amplitude of P3 and GHB ,age,course of disease respectively and a positive relation between the latency of N2 and age,course of disease and ischemic brain damage respectively. Conclusion Older persons with type 2 diabetes mellitus have cognitive impairment,and course of disease, the level of blcod glucose,complicating with ischemic brain damage may be the main influencing factors.
