国产双盘闭合伞封闭膜部室间隔缺损的近期疗效观察

摘　　要：目的评价国产双盘闭合伞封闭膜部室间隔缺损的近期疗效和安全性。方法28例膜部VSD,其中男15例,女13例;年龄5～28岁,平均年龄(13.8±5.6)岁;均行心脏超声心动图检查,测得VSD为(6.2±3.3)mm(3.6～10.6mm);术中左室造影测得VSD为(6.1±1.6)mm(4.1～10.0mm)。应用国产双盘闭合伞对上述病人经导管封闭。术中采用经胸超声监测闭合伞的释放。术后即刻、1周、1个月、3个月复查心电图、胸片、TTE。结果28例病人均一次封堵成功。其中1例超声及左室造影示少量云雾状分流,48h后复查UCG分流消失。另1例闭合伞释放后脱落,卡在右髂总动脉,再穿刺左股动脉建立轨道后封闭成功,脱落伞经手术取出;还有1例出现一过性右束支传导阻滞。术后随访3个月无心律失常、无溶血、无栓塞、无心内膜炎,封闭伞位置良好,无分流,主动脉瓣及房室瓣未受影响。结论国产双盘闭合伞封闭膜部VSD近期疗效好、安全。Objective To evaluate the short term efficacy and safty of transcatheter closure of membranous ventricular septal defect （VSD） using the amplatzer occluder device made in China. Methods There were 28 patients（ M15 ,F13 ）. The mean age of patients was （ 13.8 ± 5.6） years（ ranged from 5 to 28 years ）. Transthoracic echocardiography（ TTE ） showed the mean diameter of VSD was （6.2 ± 3.3 ）mm （ranged from 3.6 to 10.6mm）. Transcatheter left ventricular angiography showed the mean diameter of VSD was （6.1 ± 1.6）mm（ranged from 4.1 to 10.0mm）. All the patients were occluded using the amplatzer device made in China. The release of the device was measured by TTE. Follow-up was performed using EKG,chest X-ray,TTE on immediately,7 days, 1 month, 3 months. Results Successful closure was achieved in 28 patients at one time. TTE and left ventricular angiography observed a little shunt in ventricular septal in one patient, and disappeared after 48 hours. The occluder device drop occurred in one case after release, and stayed at the right iliaca communis. This device was taken out by surgery operation after successful closure the VSD through puncturing the left femoralis. One patient occurred temporal RBBB during the procedure. During the 3 months follow-up, there was no arrhythmia, no hemolysis, no thromboembolism, no endocarditis, no closure device movement, and no atrium-ventricular valve malfunction. Conclusion Transcatheter closure of membranous VSD with amplatzer occluder device made in China is safe and effective. Short term results are satisfactory.
