宫腔镜下两种手术方式治疗子宫内膜息肉的临床分析

摘　　要：目的观察宫腔镜下不同手术方式对子宫内膜息肉切除术的治疗效果。方法A组宫腔镜下刮匙搔刮并取出息肉,然后负压吸引全面吸净宫腔;B组在宫腔镜直视下以环状电极电切息肉基底部,两组比较治疗效果。结果A组:术后3个月、6个月的月经量减少,有显著性差异(P<0.05),12个月的月经量与治疗前比较,无显著性差异(P>0.05);B组:术后3个月、6个月、12个月经量均减少,有显著性差异(P<0.05)。术后妊娠情况:A组不孕症19例,术后6个月内妊娠8例,B组中不孕症26例,术后6个月内妊娠2例,两组对比有显著性差异(P<0.05);12个月内妊娠A组8例,B组妊娠7例,两组对比无显著性差异(P>0.05)。术后复发情况:6个月内A组复发4例,B组复发3例,两组比较无显著性差异(P>0.05);12个月内A组复发16例,B组复发4例,两组比较有显著性差异(P<0.05)。结论对于治疗月经量增多的子宫内膜息肉患者若不考虑生育问题,应首选宫腔镜下子宫内膜息肉电切术,而要求生育的患者宜采用刮匙搔刮并负压吸引术。Objective To evaluate two surgeries by hysteroscope for endometrial polyp. Methods A group was of dilatation and curettage, taking out the polyp and empty the uterine cavity by vacuum suction under hysteroscope; B group was electrosecting the basal part of polyp under hysteroseope, compared the difference between the two group. Results A group：menstrual blood volume decreased after 3 months and 6 months, difference was significant（P 〈 0.05 ）,while after 12 months, there was no difference （P 〉 0.05 ）. B group： menstrual blood volume decreased after 3 months, 6 months and 12 months, difference was significant（P 〈 0.05 ）, pregnance after operation： 19 cases of infertility in A group,8 cases became pregnant in 6 months, 26 cases of infertility in B group,2 cases became pregnant in 6 months,difference was significant （P 〈 0.05 ） ; 8 cases in A group while 7 cases in B group became pregnant in 12 months, there was no difference（ P 〉 0.05 ）. Recurrence： 4 cases reeured in A group and 3 cases recured in B group in 6 months,there was no difference（P〉 0.05）; 16 cases recured in A group and 4 cases recured in B group in 6 months,difference was significant（P〈 0.05 ）. Conclusion The method of electrosecting endometrial polyp by hysteroseope is a good way for the increasing of menstrual blood volume if we don＇t think about pregnancy,on the contrary,the method of scraping together with vacuum suction.
