摘 要:目的:探讨多巴胺转运蛋白(DAT)显像诊断帕金森病的价值。材料和方法:对18例正常人、31例晚期帕金森病(PD)患者分别进行11C-β-CFTPET显像,采用感兴趣区(ROI)及统计参数图(SPM)进行分析,比较正常组与PD组之间的差异。结果:ROI显示PD组尾状核及壳核各区域DAT摄取明显低于对照组,重侧基底节DAT减少较对侧明显,重侧的尾状核头、体及壳核前、中、后的区域内CFT摄取值分别为轻侧的94.6%、87.0%、89.6%、91.8%、95.4%。SPM图像显示DAT减低区域主要集中在两侧尾状核、壳核区域,以壳核中后部减低为著,重侧DAT减低区域明显大于轻侧。结论:11C-β-CFTPET显像能够客观显示帕金森病DAT不对称性代谢改变的区域,有助于帕金森病的诊断和疗效评价。SPM分析为一种更全面、准确的统计方法。Purpose: The marker of presynaptic neuron, dopamine transporter( DAT), is a sensitive indicator of neural damage in Parkinson's disease(PD). The aim of this study was to evaluate the abnormality of PD through DAT PET imaging. Materials and Methods: Thir- ty-one patients with advanced bilateral Parkinson' s disease underwent ^11C-β-CFT PET scanning. The PET finding were compared with that of 18 normal subjects. Two methods of analysis were employed: region of interest(ROI) analysis, and statistical parametric mapping (SPM). Results: ROI showed that the DAT binding in caudate and putamen in PD group was significantly reduced (P 〈 0.01 ) comparing to the normal controls, especially in the striatum of the more seriously damaged side. The DAT binding uptake in the head of caudate,body of caudate, anterior putamen,intermediate putamen and posterior putamen of the serious-side were significantly reduced to 94.6% ,87.0% ,89.6%, 91.8% ,95.4% of the counter parts of the other side of brain. In SPM analysis, reduced DAT binding was also noted in the caudate and posterior putamen. ( P 〈 0.001 corrected). The area of DAT reduction on the serious-side was larger than the contralateral side. Condusions: ^11C-β-CFT could reveal the regional change of DAT and is helpful in diagnosing Parkinson' s disease and evaluating therapeutic effect. SPM analysis is a more objective and accurate method.
