羟乙基淀粉及琥珀酰明胶对成人静脉血辅助性T淋巴细胞分化的影响

摘　　要：目的比较两种羟乙基淀粉(HES200/0.5和HES130/0.4)及琥珀酰明胶对成人静脉血辅助性T淋巴细胞(Th细胞)分化的影响。方法采集18例健康志愿者外周静脉血5 ml,采用随机区组设计,每份血样均参与下列各组研究。根据临床常用剂量设置20%、40%(体积比)两个血液稀释浓度,分为不同浓度HES200/0.5组[20%(H1组)、40%(H2组)];不同浓度HES130/0.4组[20%(V1组)、40%(V2组)];不同浓度琥珀酰明胶组[20%(G1组)、40%(G2组)];未稀释的全血的空白对照组(C组)。将全血在体外用相应剂量的药物预先培养24 h,然后加入刺激剂(氟波醇乙酯、离子霉素及蛋白质转运抑制剂),刺激4 h,采用流式细胞技术测定每份样本中Th1及Th2细胞亚群比例,并计算Th1与Th2细胞亚群的比值(Th1/Th2)。结果与C组比较,H1、V1组Th1细胞亚群比例升高(P<0.05或P<0.01),H2、V2组Th2细胞亚群比例下降(P<0.05或P<0.01);G2组Th1、Th2细胞亚群比例均下降(P<0.01);H2组、V1组、V2组、G2组Th1/Th2比值均升高(P<0.05或P<0.01)。H1、V1、G1组三组的Th1、Th2细胞亚群比例分别高于H2、V2、G2三组(P<0.05或P<0.01)。结论在20%浓度时,HES200/0.5、HES130/0.4可促使Th细胞向Th1细胞亚群分化,琥珀酰明胶作用不明显;在40%浓度时,HES200/0.5、HES130/0.4抑制Th2细胞亚群的分化,琥珀酰明胶对Th1和Th2细胞亚群的分化均有抑制作用。HES130/0.4在两种浓度下均可引起Th1/Th2比值升高。Objective To investigate the effects of pretreatment with different concentrations of hydroxyethyl starch （HES200/0.5 and HES180/0.4） and succinylated gelatin on the differentiation of human helper T cells in vitro. Methods Eighteen healthy volunteers were enrolled in this study. Two clinically relevant degrees of in vitro haemodilution were prepared： 200/00 and 40% （V/V）. In each volunteer, 5 ml of blood was taken from peripheral vein and divided into 7 groups： two HES200/0. 5 groups （H1, H2）, two HES130/0.4 groups （V1, V2）, two succinylated gelatin groups （G1, G2）, stimulation control group （C）. Whole blood was pretreated with different colloids. To estimate the imbalance of Th1/Th2, the intracellular eytokines （IFN-7/IL-4） were detected by four-color flow cytometry. Results In HI and V1 groups the percentages of Thl cells increased significantly,while the percentages of Th2 cells decreased in H2 and V2 groups. In G2 group the percentages of both Thl and Th2 cells decreased significantly. The ratios of Thl/Th2 were increased in H2,V1 ,V2 and G2 groups. Conclusion HES200/0.5 and HES130/0.4 encouraged the helper cells to differentiate into Thl cells while succinylated gelatin inhibited the differentiation of Th1 and Th2 cells. HES130/0. 4 can increase the Th1/Th2 ratios.
