超声二维斑点跟踪显像技术评价2型糖尿病患者左室扭转运动的临床研究

摘　　要：目的应用超声二维斑点追踪显像技术(STI)探讨2型糖尿病患者左室心肌扭转运动特点。方法血糖控制良好的2型糖尿病患者36例,其中左室舒张功能正常组16例,左室舒张功能减低组20例;正常对照组21例。应用STI技术对3组研究对象的左室扭转进行定量分析。结果血糖控制良好的2型糖尿病患者和正常人的左室扭转角度、扭转速度曲线形态基本一致;糖尿病患者的左室扭转角度峰值较正常组明显升高,解旋率及解旋减半时间明显减低和延迟,程度随舒张功能的下降而加剧。结论TSI技术可以早期识别2型糖尿病患者的心功能异常变化。Objective To evaluate the left ventricular twist （LVtw） in patients with type 2 diabetes using 2- dimensional ultrasound speckle-tracking imaging （STI） Methods Thirty-six patients with type 2 diabetes and 21 control people underwent the examination of two-dimensional ultrasound. Diabetic patients were divided into two groups according to left ventricular diastolic function on tissue Doppler imaging （normal diastolic function group included 16 patients and abnormal diastolic function group included 20 patients）. The standard LV basal and apical short-axis views were acquired in the three groups, and the mean value of LVtw was obtained using STI. LV twist and twist velocity was defined as apical rotation/rotation rate relative to the base. Results Diabetic patients had the same shape as the control in the LV torsion curve and LV torsion velocity curve, but they had a higher peak LV torsion than the normal group, no matter their diastolic functions were normal or not. On the other hand, compared with normals, the rate of LV untwisting in diabetic patients was significantly reduced （P〈0.01）, the time to peak untwisting velocity were significantly delayed （P〈0. 01）, and the difference were more obvious in the abnormal diastolic function sub-group. Conclusions LV twist can be assessed noninvasively on 2-dimensional ultrasound STI imaging, STI could evaluate the early changes of LV function in patients with type 2 diabetes.
