摘 要:目的通过应用血管内皮生长因子(VEGF)中和抗体,探讨VEGF在屈肌腱愈合中的作用机制。方法制作兔屈肌腱损伤模型,局部应用VEGF中和抗体,分别于术后4,7,10,14,28,56d6个时相点取材,大体和组织学观察应用VEGF中和抗体对肌腱粘连评分、VEGF的表达、胶原合成的影响,对比应用抗体前后的肌腱生物力学,透射电镜观察修复区细胞超微结构。结果应用中和抗体后,肌腱粘连程度评分、VEGF的表达水平和肌腱的位移降低,但对胶原肌腱的断裂载荷及细胞超微结构无明显影响。结论VEGF可能主要是通过肌腱的外源性愈合发挥作用,局部应用VEGF中和抗体可以抑制外源性愈合,减轻肌腱粘连。Objective To explore the mechanism of vascular endothelial growth factor (VEGF) in promoting flexor tendon healing by applying neutralizing antibody of VEGF. Methods Flexor tendon injury models were made and given VEGF locally. The specimens were harvested respectively at days 4, 7, 10, 14, 28 and 56 after operation. The influence of neutralizing antibody of VEGF on tendon adhesion grade score, expression level of VEGF and collagen synthesis was observed by gross and histological methods. The biomechanics before and after application of neutralizing antibody of VEGF was compared. Meanwhile, ultrastructure of the cells was also observed by transmission electron microscope. Results After neutralizing antibody was given, the tendon adhesion grade score, expression level of VEGF and offset of the tendon were degraded, but there was no change of collagen synthesis, breakage load of the tendon and ultrastructure of the cells. Conclusion VEGF takes effect in flexor tendon healing mainly through extrinsic healing. Local use of neutralizing antibody of VEGF may inhibit extrinsic healing and relieve tendon adhesion.
