摘 要:目的探讨新生儿缺氧缺血性脑病(HIE)外周血T细胞亚群、IL-2及神经元特异性烯醇化酶(NSE)的动态变化,为新生儿HIE的诊断、分度、治疗及预后提供新的理论依据。方法选择36例围生期有缺氧缺血性脑病的新生儿为研究对象(HIE组),同时选择20例同期出生的正常足月儿为对照组,分别检测T细胞亚群CD3、CD4、CD8、CD4/CD8比值、IL-2和血清NSE的动态变化。结果①急性期HIE组T细胞亚群CD3、CD4、CD4/CD8比值及IL-2水平低于正常对照组(P<0.01),CD8及NSE水平高于对照组(P<0.01);②不同程度HIE组急性期T细胞亚群、IL-2及NSE水平之间相比有显著差异性(P<0.01);③恢复期T细胞亚群、IL-2及NSE水平与正常对照组之间相比仍有显著性差异(P<0.01),和急性期相比随病情的好转各项指标有明显改善(P<0.01);④T细胞亚群CD3、CD4、CD4/CD8比值、IL-2与NSE呈负相关,CD8T细胞亚群与NSE呈正相关。结论①新生儿HIE存在着免疫功能紊乱;②T细胞亚群、IL-2及NSE参与了新生儿HIE的病理生理过程;③外周血CD8T细胞亚群和NSE水平升高,CD4/CD8比值和IL-2水平下调,与新生儿HIE轻重程度呈正相关。Objective To evaluate the dynamic changes ofT cell subgroups, IL-2 and NSE levels in the newborn babies with HIE and its clinical significance, and to provide new evidence for the diagnosis, therapy and prognosis of HIE. Methods To select 36 cases of infants with HIE in perinatal period and they were divided into three groups depending on the severity of the patients'condition, and at the same time to select 20 cases of healthy newborn babies as control group. To collect peripheral venous blood in acute stage and restoration stage of the HIE group . Two blood samples were collected in each cases, one is on the first to third day after birth, the other on the seventh to tenth day after the treatments. T cell subgroups CD3, CD4, CD8, CD4/CD8,IL-2 and NSE were measured differently by FC, RIA and ECLI. Results ①In acute stage, HIE group's CD3, CD4, CD4/CD8 and IL-2 were lower than the control group g, while CD8 and NSE higher than the control group ; and they had significant difference between HIE group and control group { P 〈 0. 01 ). ②There was significant difference between the levels of T cell subgroups, IL-2 and NSE with different severity in acute stage of HIE { P 〈 0. 01 ). ③There was significant difference between the levels of CD3, CD4, CD4/CD8, IL-2 and NSE in the restoration stage with HIE and the control group { P 〈 0. 01 ). ④The concentrations of CD3, CD4, CD4/CD8, IL-2 had negative correlation with NSE, while CD8 positive correlation with NSE. Conclusion It is suggested that immunity function was impaired and disodered in neonates with HIE. T cell subgroups, IL-2 and NSE may be involved in the pathophysiology course of HIE. T cell subgroups, IL-2 and NSE were correlated with the severity of HIE, the more serious of lower level of T cell subgroups and IL-2, the higher level of NSE.
