摘 要:目的探讨三维彩色血管能量成像(3D-CPA)联合术后肿瘤标本组织内的微血管密度(micro-vasculardensi-ty,MVD)测量对鉴别卵巢肿瘤良恶性的临床价值。方法2003年12月至2005年5月对中国医科大学附属盛京医院43例临床怀疑为卵巢肿瘤的患者进行3D-CPA,术后应用抗血管内皮生长因子CD34抗体,利用免疫组化SP染色法对病理组织切片进行免疫组织化学染色,计数肿瘤的MVD。结果在良恶卵巢肿瘤中MVD的表达有明显的差异性,恶性肿瘤的MVD定量表达明显高于良性肿瘤(P<0.001),3D-CPA在鉴别诊断恶性卵巢肿瘤方面有较高的特异性,与病理诊断结果的符合率(准确性)可达81.4%,但却有比较高的假阳性率。结论3D-CPA对于恶性卵巢肿瘤的定性诊断有很高的特异性。在鉴别诊断卵巢肿瘤良恶性方面3D-CPA与MVD具有良好的一致性,可以作为术前检查应用于卵巢肿瘤良恶性的诊断。Objective To evaluate the clinical efficacy about the 3D color power angiography (3D-CPA) and micro-vascular density (MVD) in differentiation of benign and malignant ovarian tumor by classifying the tumor vessels qualitatively using 3 D-CPA imaging technique and after the operation counting the MVD of the specimens. Methods We examined 43 women who were suspected acquiring ovarian tumor using 3D-CPA imaging technique to classify the tumor vessels qualitatively, and then made the diagnosis of benign or malignant. After the operation, we dyed the specimens with the antibody of vascular endothelial growth factor CD34 and by using immunohistological SP stain we dyed the pathological slices to count the MVD. Results Great differences existed in the expression of MVD in benign and malignant ovarian tumor. The malignant tumor expressed apparently more than the benign one (P 〈 0. 001 ). The results illustrated the high specificity of 3D-CPA in differentiating benign and malignant ovarian tumor, as it had an accordance of 81.4% with the pathological findings; however, it also had a high false-positive rate. Conclusions 3 D-CPA has a high specificity in the qualitative diagnosis of the ovarian malignant tumors. In the differential diagnosis of benign and malignant ovarian tumor, 3D-CPA acquires the same efficacy as the MVD. 3D-CPA could be used to indirectly evaluate the tumor vascularization before the operation and it is an effective differential diagnosing method in clinics.
