摘 要:目的探讨空腹TC水平对代谢综合征患者餐后脂代谢的影响。方法将40例代谢综合征患者分为空腹TC水平升高(≥1.70mmol/L)组(MS+HTG组,24例)和空腹TC水平正常(〈1.70mmol/L)组(MS—HTG组,16例),同时将20例健康者作为对照组。三组受试者分别进行口服脂肪容许量测试,于餐后2、4、6、8、10h测定TG水平,并计算其餐后TG代谢的曲线下面积(AUC)。结果MS+HTG组餐后TG的高峰水平和餐后TG代谢的AUC[(6.83±2.81)mmol/L和(26.08±16.40)mmol/(L·10h)]均显著高于MS—HTG组[(4.12±1.14)mmol/L和(12.71±7.05)mmol/(L·10h)]及对照组[(3.14±1.26)mmol/L和(10.28±6.09)mmol/(L·10h)]。MS+HTG组餐后TG的高峰水平较其他两组延迟2h。MS—HTG组餐后TG的高峰水平高于对照组(P〈0.05),MS—HTG组餐后TG代谢的AUC虽高于对照组,但差异无统计学意义。相关分析结果表明,餐后TG代谢的AUC与空腹TG水平及体重指数(BMI)相关(r=0.60,P〈0.01;r=0.27,P〈0.05)。多元线性回归分析结果显示,仅空腹TG水平对餐后TG代谢的AUC有预测价值(P〈0.01)。结论代谢综合征患者的空腹TG水平及BMI与餐后高TC血症独立相关。空腹TC水平对餐后高TC血症有预测价值。空腹TC水平越高,餐后TG的清除越迟缓。Objective To study the effect of fasting triacylglycerol (TG) level on postprandial lipid metabolism in metabolic syndrome (MS). Methods Orally fat tolerance test (OFTT) was given to 40 MS subjects who were divided into MS±HTG group (TG≥ 1.70 mmol/L, 24 cases ) and MS-HTG group (TG 〈 1.70 mmol/L, 16 cases) according to fasting TG and 20 healthy subjects (control group). Levels of TG were measured before and 2,4,6,8 and 10 hours after OFTT. The postprandial response and clearing was quantified by area under the curve (AUC) of TG level. Results Compared with MS-HTG group and control group, MS±HTG group had higher peak level [ (6.83 ±2.81 )mmol/L vs (4.12 ± 1.14) mmol/L and(3.14 ± 1.26) mmol/L] and higher AUC [(26.08 ± 16.40) mmol/(L·10 h) vs (12.71 ± 7.05) mmol/(L·10 h) and (10.28 ± 6.09) mmol/ (L· 10 h)]. The peak level of postprandial TG in MS+HTG group appeared 2 hours later than that in other two groups. The MS-HTG group had higher peak level compared with control group (P 〈 0.05). The AUC in MS-HTG group was higher than that in control group,but no significant difference was shown. In multiple correlation analysis,AUC was correlated with fasting TG level(r = 0.60,P 〈 0.01 ) and BMI (r = 0.27, P 〈 0.05 ). In linear regression analysis, only fasting TG was a significant predictor of the AUC(P 〈 0.01 ). Conclusions The fasting TG level and BMI is correlated with postprandial hypertriglyceridemia independently. The fasting TG level is the main determinant of postprandial lipemia. The higher fasting TG level, the later clearance of postprandial TG.
