摘 要:目的:评价多种MRI序列对髌骨软骨病诊断的敏感性,特异性和准确率,提出其中的优选序列。材料和方法:对50例患者51个髌骨进行术前MRI检查,男性23例,女性27例28个髌骨,其中37例38个髌骨行关节镜检查,13例行手术治疗,以关节镜或手术结果为金标准行双盲前瞻性分析。结果:对髌骨软骨病的诊断,TSE-T1W,TSE-T2W,STIR,T1-FFE三个序列的敏感性分别是:50%,413%,455%,846%,特异性分别是:8823%,98%,981%,745%,准确率分别是:627%,6209%,627%,81%。结论:T1-FFE是三个序列中的优选序列。MRI是诊断髌骨软骨病最优选影像学方法。PURPOSE:To evaluate the sensitivity,specificity and accuracy of multiple MR pulse sequence in diagnosing chondropathy patellae for sorting out the optimal pulse sequence. MATERIALS AND METHODS:This study covered 51 patellae of 50 patients. MR Imaging was performed for them befere operation. Among them 23 were males and 27 were females with 28 patellae. Arthroscopy was used in 38 knees,while the rest underwent arthrotomy. The gold standards were the results of arthroscopy or arthrostomy. The methods of analysis were two blinds and prespective. RESULTS:For the diagnosis of chondropathy patellae, the sensitivity of TSE-T 1W,TSE-T2W,STIR,T1-FFEwas 50%,41 1%,45 5%,84 6% respectively. The specificity was 88 23%,98%,98 1%,74 5% respectively. The accuracy was 62 7%,62 09%,62 7%,81% respectively. CONCLUSION: T1-FFE was the optimal sequence in diagnosing chondropathy patellae. MRI was one of the best imaging methods.
