早期梅毒螺旋体感染最适检测方法的探究

摘　　要：目的:探究早期梅毒螺旋体感染的最适检测方法。方法:采用实时荧光定量聚合酶链反应(FQ-PCR)、梅毒螺旋体血凝试验(TPHA)、梅毒螺旋体酶联免疫吸附试验(TP-ELISA)、快速血清反应(RPR)法对102例早期疑似梅毒患者、30例非梅毒患者分泌物、血清进行梅毒螺旋体(TP)检测,比较4种检测法灵敏度及特异性。结果:102例早期疑似梅毒患者FQ-PCR检出92例检出率90.20%,TPHA检出86例检出率84.31%,TP-ELISA检出88例检出率86.27%,RPR检出70例检出率68.6%。30例非梅毒患者FQ-PCR检测全阴性,TPHA检出2例阳性,TP-ELISA检出1例,RPR检出6例阳性。结论:早期梅毒患者诊断首选FQ-PCR进行分泌物或溃疡创面渗出物检测TP-DNA,其次选用TPHA和TP-ELISA进行血清学检测,RPR在梅毒感染早期检出率较低,易出现漏检现象。Objective：To investigate early syphilis infection.Methods：A total of 102 suspected cases of early syphilis patients and 30 patients with non-secretory syphilis were detected by real-time quantitative polymerase chain reaction（FQ-PCR）,treponema pallidum hemagglutination test（TPHA）,treponema pallidum enzyme-linked immunosorbent assay（TP-ELISA） serum rapid reaction（RPR） and serum treponema pallidum（TP） detection.Comparing the four detection sensitivity and specificity.Results：102 cases of early syphilis in patients with suspected FQ-PCR Detection of 92 cases detectable rate of 90.20%,TPHA detectable 86 cases detectable rate 84.31%.TP-88 is detectable by ELISA and detection rate of 86.27%,RPR detectable 70 cases detectable rate 68.6%.30 cases of syphilis patients with non-FQ-PCR all negative,TPHA detectable two cases positive,TP-1 is detectable by ELISA,RPR detectable positive six cases.Conclusion：Early diagnosis of syphilis patients preferred FQ-PCR secretions positive six cases.Conclusion：Early diagnosis of syphilis patients preferred FQ-PCR secretions or ulcer leakage detection TP-DNA,Second choice TPHA and TP-ELISA for serological testing,RPR in the early detection of syphilis infection rate lower,easily missed phenomenon.
