摘  要:目的探讨热休克蛋白70表达在过敏性紫癜发病机制中的作用及其与临床的关联。方法采用逆转录聚合酶链反应(RT-PCR)方法检测了66例过敏性紫癜患者和30例正常对照组外周血单一核细胞中热休克蛋白70表达,采用免疫速率散射比浊法检测患者血沉、C反应蛋白和血清IgA。结果过敏性紫癜患者治疗前热休克蛋白70的表达明显高于治疗后和对照组,其在肾型表达明显高于其他类型,治疗后其表达与对照组无明显差异,治疗前热休克蛋白70表达与血沉、C反应蛋白、血清IgA明显正相关,其表达和上呼吸道感染、腹痛、关节痛、蛋白尿、血尿明显相关。结论热休克蛋白70表达在过敏性紫癜活动期明显升高,恢复期则明显下降,且与临床表现密切相关,提示其在该病发生发展中具有一定的作用,可作为疾病活动的指标之一。Objective To investigate the role of the expression of heat shock protein 70 in the pathogenesis of anaphylactoid purpura and its relationship with clinical manifestations. Methods Expression of heat shock protein 70 was detected by reverse transcription polymerase chain reaction (RT-PCR), ESR, C-reactive protein and serum IgA levels were tested by Immuno-Velocity Scattering Turbidimetry, Results Expression of heat shock protein 70 in patients with anaphylactoid purpura was significantly higher than those after treatment and controls. And its expression in patients with renal involvement was significantly higher than those with other' s types. Its expression wasn' t significantly different between those after treatment and controls, and was positively associated with ESR, C-reactive protein and serum IgA levels. The elevation of its expression was also significantly associated with proteinuria and hematuria. Conclusion HSP70 is elevated in the initial active stage and decreased in the recovery stage of ana- phylactoid purpura, associated with clinical features, suggesting that it may play some role in the onset and progressive of anaphylactold purpura. We believe that serum HSP70 might be an indicator of anaphylactoid purpura' activity.
