摘 要:目的探讨高胸段硬膜外阻滞(HTEA)对兔缺血/再灌注心肌C—jun氨基末端激酶(C-iun—N—terminalki—nase,JNK)和p38磷酸化水平的影响。方法 24H日水火白兔,雌雄不限,体重2kg~3kg,随机分为4组,每组6只:正常组(N组,开胸后直接取心脏备用),假手术组(S组,开胸冠脉置线观察后取心脏备用),实验组(E组:缺血/再灌注+硬膜外注入1%利多卡因(2mg/kg~3mg/kg),对照组(c组:缺m/再灌注+硬膜外注入牛理盐水0.2ml/kg~0.3ml/kg)。采用结扎冠脉左旋降支(LAD)45min,再灌注2h制作心肌缺血/再灌注模型,实验结束取心尖部心肌组织采用Western—blot法测定心肌细胞JNK和p38磷酸化水平。结果正常组和假手术组组间各指标没有统计学差异(P〈0.05);对照组.INK和p38磷酸化水平都明显高于正常组和假手术组(P〈0.01);实验组JNK和p38磷酸化水平与正常组和假手术绁相比没有明显变化(P〉0.05),与对照组比明显降低(P〈0.01)。结论HTEA可以抑制缺血/再灌注兔心肌细胞JNK和p38磷酸化水平。Objective To evaluate the effect of HTEA on .Jur N - terminal kinase (JNK) and p38 phosphorylation of ischemia/ reperfusion myocardium in rabbit. Methods Rabbits of both sexes weighing 2 kg to 3 kg were randomly divided into four groups (n = 6 each). Animals in group N did not get any treatment, while in group S the left anterior descending coronary artery (LAD) was not ligated. Myocardial ischemia/reperfusion models were established in group E and C. Animals in group E received 2 mg/kg to 3 mg/kg of epidural 1% Lidocain while in group C received 0.2 ml to 0.3 ml of normal saline. The rabbits of group E and group C were anesthetized intravenously with 20% uretham before tracheal intubation. After the success of T6-7 epidural puncture, a catheter was advanced epidurally 3 cm-4 cm. Femoral artery was cannulated to determine MAP and then the LAD was dissected. 2 mg/kg to 3 mg/kg of 1% epidural Lidocain was injected in the group E and 0.2 ml/kg to 0.3 ml/kg of epidural normal saline was injected in the group C. After 15 min, the LAD was ligated and 45 min later it was set free. During the operation, Lidocain and normal saline were injected every 30 min. 2 h later the myocardium of the cardiac apex was taken to determine the p - JNK and p - p38 by Western - blot. Results there was no prominent difference between group N and group S ( P 〉 0.05 ). Compared with group N and group S, there were prominent increase of p -JNK and p -p38 in group C( P 〈 0.01 ). Compared with group N and group S, p -JNK and p -p38 in group E had not significant difference (P 〉 0.05 ), but sharply decreased as compare with group C (P 〈 0.01 ). Conclusion HTEA markedly decreased phosphorylation of JNK and p38 in ischemia/reperfusion myocardium of rabbit.
