摘  要:目的探讨新生儿缺氧缺血性脑病(HIE)患儿血小板活化因子(PAF)和血小板参数的变化及临床意义。方法对96例HIE患儿于急性期进行PAF及血小板数总数(PLT)、血小板平均体积(MPV)及其分布宽度(PDW)测定,恢复期进行复查,了解其动态变化。结果①HIE患儿急性期血PAF水平较对照组明显升高,且血PAF含量随病情加重而升高;恢复期中重度HIE患儿PAF仍高于对照组。②HIE患儿急性期血小板总数较正常儿明显降低(P〈0.01),MPV和PDW较正常儿明显增高(P〈0.01)。恢复期BPC、MPV和PDW与正常组之间差异无统计学意义(P〉0.05),HIE患儿PLT随HIE病情加重而降低(P〈0.01),而MPV和PDW则随病情加重而增大(P〈0.01)。结论动态观察HIE患儿PAF及血小板参数,可判断HIE病情的严重程度。Objective To study the changes and the significance of the blood platelet-activating factor (PAF) levels and platelet parameter in neonatal hypoxic-ischemic encephalopathy (HIE). Methods The levels of blood PAF, the platelet total ( PLT ), mean platelet volume ( MPV ) and platelet distribution width ( PDW ) were measured in neonates with HIE and compared with that of normal term neonates. Results Blood PAF levels were much higher in acute stage of HIE, and PAF levels increased with the severity of HIE. In the recovery period PAF levels in the moderate and severe group were still higher than that in the control group during the recovery period. The value of PLT in each group of HIE in the acute period was obviously lower than that in the control group ( P 〈 0.01 ) , but the values of MPV and PDW were obviously higher than those in the control group ( P 〈 0.01 ). The values of PLT, MPV and PDW in each group of HIE in the recovery period returned to normal, compared with the control group, there was no significant difference ( P 〉 0.05 ). Conclusion Observing the dynamic changes of PAF and platelet parameter in neonates with HIE is conducive to judge the severity of HZE.
