血清基质金属蛋白酶-2水平对判断心力衰竭患儿预后的临床价值

摘　　要：目的探讨心力衰竭(HF)患儿血清基质金属蛋白酶-2(MMP-2)水平变化及临床意义。方法2005年3月至2007年3月于山东省济南市第四人民医院儿科住院的慢性心力衰竭(CHF)患儿51例(HF组),同时选该院门诊体检健康儿童30名作正常对照。测定HF组和对照组的血清MMP-2和N末端脑钠肽(NT-proBNP)水平、左心功能参数,同时分析HF组血清MMP-2与NT-proBNP、左心功能参数之间的相关性。结果HF组血清MMP-2水平显著高于对照组,且随着HF程度的加重而显著升高(P<0.01);MMP-2与NT-proBNP之间具有良好的相关性(r=0.78,P<0.01)。结论血清MMP-2水平的检测有助于动态评估HF患儿病情的严重程度及预后。Objective To investigate the changes and clinical significance of serum matrix metalloproteinase-2 （ MMP- 2 ） concentration in the children with heart failure （HF）. Methods Serum MMP-2, plasma N-terminal pro-brain natriuretic peptide （NT-proBNP） concentrations and left ventricular parameters were all measured in HF group and control group,and the relationship between serum MMP-2 and NT-proBNP,left ventricular parameters were also studied. Results The serum MMP-2 concentration of patients with HF were significantly higher than the control group, and it also increased with the increasing severity of HF （ P 〈0. Ol ）. The serum MMP-2 concentration and plasma NT-proBNP had significant correlations （ r = 0. 78, P 〈 0. 01 ）. Conclusion Serum MMP-2 can be involved in left ventricular remodeling of HF and measuring its concentration is helpful to judging the severity and prognosis of HF.
