摘　　要：目的探讨原位心脏移植手术患者长期、大剂量使用糖皮质激素和免疫抑制剂后眼部并发症的特点,为早期诊断、及时治疗提供依据。方法前瞻性随访2000年5月至2005年10月行原位心脏移植手术的患者138例,对眼部病变病程、症状、体征、全身药物使用情况进行分析。结果所有138例(276眼)中,外眼疾病47眼(17.0%),后囊膜下白内障64眼(23.2%),激素性青光眼16眼(5.8%),眼底病变15眼(5.4%)。结论心脏移植患者术后可出现多种眼部病变,主要与长期使用激素和免疫抑制剂有关。心脏移植术后患者的眼部早期症状和体征应引起心脏外科和眼科医师的高度关注。Objective To investigate the characteristics of ocular complications in patients after long-term hight and corticosteroid treatment for the maintenance of orthotopic heart transplants. To provide reference for early diagnosis and treatment for this kind of eye diseases. Methods A prospective analysis was done in 138 patients with orthotopic heart transplantation from May 2000 to October 2005, including ophthalmic symptoms, signs and general conditions. Results Of the 138 transplant recipients （276 eyes） examined, 47 eyes （ 17.0% ） had ocular surface diseases, 64 eyes （23.2%） had posterior subcapsular cataract, 16 eyes （5.8%） had corticosteroid glaucoma and 15 eyes （5.4%） had ocular fundus diseases. Conclusions Many kinds of ocular complications due to immunosuppressant and corticosteroid usage might appear in heart recipients. Cardiac surgeons and ophthalmologists should pay more attention to ocular symptoms in the post-transplant period.
