标准大骨瓣减压并天幕切开治疗颅脑损伤中晚期天幕疝

摘　　要：目的提高颅脑损伤中晚期天幕疝的手术治疗效果。方法回顾性分析21例颅脑损伤中晚期天幕疝采用标准大骨瓣减压并天幕切开手术治疗患者的临床资料。结果21例患者生存18例,死亡3例。术后随访2年,恢复良好11例,中残5例,重残2例,重残及死亡率23.8%。结论标准大骨瓣减压并天幕切开可以有效减压,尤其对于合并脑干损伤的患者,可以提高生存率,改善预后。Ojective To improve curative effect of traumatic brain injury with metaphase and advanced stage herniation, Methods A retrospective analysis was performed on the clinical data of 21 patients with typical big decompressive craniectomy and discissio temtproum of cerebellum. Results Among the 21 cases, 17 cases live, 3 cases died , 11 cases survived well, 5 cases（23.8% ） suffered from moderate disablity ,2 cases（9.5% ） severe deficit, Conclusion Typical big decomprossive cranlectomy and discissio tentorium of cerebellum could effective decompression, Especially for the brain -stem injuries,this could help to upgrade the survival rate and improvement prognosis.
