肺结核病人疱疹病毒感染的检测和临床意义

摘　　要：目的探讨肺结核病人疱疹病毒活动性感染情况。方法采用酶联免疫吸附试验(ELISA),检测62例肺结核病人和43例正常对照者血清中人巨细胞病毒(HCMV)和单纯疱疹病毒(HSV)特异性IgM抗体以及EB病毒(EBV)衣壳抗原(VCA)IgM抗体。同时,采用PCR-Southern检测肺结核病人和正常人外周血单个核细胞中EBV DNA表达。结果肺结核组血清HCMV-IgM和HSV-IgM阳性率分别为19.4%和12.9%,正常对照组分别为4.7%和0,肺结核组HCMV-IgM和HSV-IgM阳性率均高于正常对照组(χ2=4.75、4.31,P<0.01)。肺结核组和正常对照组均未检测到EB病毒VCA-IgM,肺结核组EBV DNA阳性率(48.4%)高于正常对照组(4.7%),差异有显著性(χ2=22.92,P<0.01)。继发性肺结核疱疹病毒感染率(39.6%)高于原发性肺结核(7.1%),差异有显著性(χ2=3.84,P<0.05)。结论部分肺结核病人存在HCMV、HSV以及EBV的活动性感染,人疱疹病毒感染可能影响肺结核病情发生发展。Objective To explore the active infection of human herpesvirus in patients with tuberculosis （TB）. Methods The specific IgM antibodies against human cytomegalovirus （HCMV）, herpes simplex virus （HSV） and VCA of Epstein- Barr virus （EBV） in serum of 62 TB patients and 43 normal controls were detected by ELISA, and the expression of EBV DNA in mononuelear cells in peripheral blood of both groups by PCR-Southern. Results The positive rates of anti-HCMV and anti-HSV IgM in the patients were 19.4% and 12.90%, respectively,and that in the control groups were 4.7% and 0. The positive HCMV- IgM and HSV-IgM in the patients were higher than that in the controls （χ2= =4.75,4. 31; P〈0. 01）. However, VCA-IgM of EBV was not detected in both groups. The positive rate of EBV-DNA in the patients （48.4%） was significantly higher than that of the controls （4.7%,χ2= 22. 92,P〈0. 01）. The infection of herpesvirus was higher in secondaryTB （39.6%） than primary （7.1%）, the difference was significant （χ2= 3.84,P〈0. 05）. Conclusion Active infection of HCMV, HSV and EBV exists in some TB patients, which suggests that human herpesvirus infection might impact the pathogenesis of TB.
