患者因素与根管治疗期间急症相关性的临床研究

摘　　要：目的对根管治疗期间急症（IAE）的患者因素进行临床评价。方法对3801例患者（共4062颗患牙）进行临床评价，分别分析性别、年龄、系统性疾病，过敏体质，患牙的解剖位置，牙髓状况，治疗前症状，有无瘘道和根尖阴影等因素。结果IAE发生率女性〉男性，40～60岁阶段发生率最高，而〈20岁的患者发生率最低，有系统疾病〉无系统疾病，有过敏体质〉无过敏体质，下颌牙〉上颌牙，磨牙〉前磨牙〉前牙，牙髓无活力〉牙髓有活力，治疗前有症状〉治疗前无症状，根尖无阴影〉根尖有阴影，其差异均有统计学意义（P〈0．05）。结论IAE的发生与患者体质，患牙的解剖位置，患牙病情有关，临床操作中应予以重视。Objective To evaluate the correlation between patients＇ factors and endodontic interappointment emergencies（IAE） by clinical observation. Methods Evaluated the factors of 3801 patients（4062 teeth） ,including distlnction,age,systematic disease,allergic constitution, tooth location, pulp status, pretxeatment pain, fistula and periapical radiolucency. Results The occurrences of IAE under these factors had significant defferences（ P 〈 0.05 ） and the following are the typical ones. Female 〉 Male ;40-year-old to 60-year-old had the highest occurrences and the under-20-year-old had the lowest;patients who had systematic diseases and allergic constitution had higher occurrences ; Mandibula 〉 Maxillary ; molar teeth 〉 premolar teeth 〉 anterior teeth ; novital pulp 〉 vital pulp ; patients who had pretreatment pain had higher occurrences ; presence of periapical radiolucency 〉 absense of periapical radiolucency. Conclusion The occurrences of IAE have correlations with patients＇ constitutions, teeth location and teeth status. While RCT operating, they should be highly respected.
