彩色多普勒超声在乳腺肿块鉴别诊断中的应用

摘　　要：目的探讨彩色多普勒血流显像鉴别乳腺肿块的诊断价值。方法对89个乳腺肿块(良性肿块组47个,恶性肿块组42个,均经术后病理证实)进行超声检查,对比分析良、恶性肿块的二维超声特点、彩色血流情况及血流动力学参数的特征。结果良性肿块组强回声晕显示率为19.1%,明显低于恶性肿块组的66.7%(P<0.001);良性肿块组钙化显示率为17.0%,明显低于恶性肿块组的83.3%(P<0.001);乳腺恶性肿块中钙化点多呈簇状分布(27/35),而良性肿块中钙化点多呈斑点状分布(6/8),2组在钙化形态上比较也有显著性差异(P<0.05)。良性肿块组的收缩期峰值流速(PSV)为(9.21±3.86)cm.s-1,阻力指数(RI)为0.67±0.12,搏动指数(PI)为1.09±0.31,明显低于恶性肿块组的PSV(15.1±8.73)cm.s-1,RI 0.88±0.10,PI 1.68±0.41,2组之间比较有显著性差异(P<0.001)。结论彩色多普勒超声作为无创的检查方法,可清晰显示肿块的二维声像图特点和血流特征,对乳腺肿块的鉴别均有重要临床应用价值。Objective To investigate the value of color Doppler ultrasound in differential diagnosis breast lump. Methods Eighty-nine breast lumps （benign group was 47cases ,malignant was 42 cases ,they all identified by histopathology ） were detected by color Doppler ultrasound before operation and the characteristics on 2-dimensional ultrasound, the condition of colour blood and hemodynamics parameter were compared between benign and malignant lumps. Results The detection rate of hyperechoic halo in benign group（ 19.1% ） was significantly lower than that in malignant group（66.7% ） （P 〈 0. 001 ）. The detection rate of calcification in benign group（ 17.0% ） was significantly lower than that in malignant（ 83.3% ） （P 〈 0.001 ）. The calcification showed cluster distribution in malignant lumps （27/35）, and showed spot distribution in benign, lumps （5/7）. There was significant difference in the shape of calcification between malignant group and benign group （ P 〈 0.05 ）. Peak systolic velocity （PSV） , resistance index （RI） , pulsatility index （PI） in benign group （ 9.21 ± 3.86 cm·s^-1,0.67 ± 0.12,1.09 ± 0.31, respectively） were lower than those in malignant group （ 15.1 ± 8.73 cm·s^-1,0.88 ± 0. 10, 1. 68 ± 0. 41, respectively）. There was obvious difference between two groups（ P 〈 0. 001 ）. Conclusion Color Doppler ultrasound as a noninvasive examinational method can clear display the characteristics on 2-dimensional ultrasound and blood of breast lump. It plays an important role in the differential diagnosis of breast lump.
