摘 要:【目的】评价64层螺旋CT血管成像对主动脉夹层的诊断价值。【方法】47例临床诊断或可疑诊断为主动脉夹层的患者,采用Bolus Tracking自动触发扫描行64层螺旋CT血管成像检查,利用多种后处理技术进行主动脉全程重建并分析病变。【结果】47例患者中Ⅰ型主动脉夹层9例,Ⅱ型主动脉夹层4例,Ⅲ型主动脉夹层34例。其中21例Ⅲ型主动脉夹层与DSA主动脉造影对照,64层螺旋CT共显示破口52个,DSA显示破口44个。13例行覆膜支架破口封堵术,64层螺旋CT复查发现Ⅰ型内漏4例。【结论】64层螺旋CT以其快速、薄层的容积扫描和多种图像后处理技术,能清晰、真实、全方位、多角度的显示主动脉全程,在对主动脉夹层的确诊、准确分型、治疗方案的选择、术前、术后评价等方面具有重要的临床应用价值,是一种安全、无创、快速、准确的检查诊断方法。[Objective] To evaluate the value of 64 slices spiral CT angiography in the diagnosis of aortic dissection. [Methods] Forty-seven patients with known or suspected aortic dissection were underwent 64 slices spiral CT angiography using Bolus Tracking auto-scan, the full aortery and aortic dissection were reconstructed and analyzed with all kinds of management technology. [Results] Among 9 of 47 cases were type I aortic dissection , 4 cases type Ⅱ, 34 cases type Ⅲ. Among 21 cases type Ⅲ aortic dissection underwent both 64 slices spiral CT and DSA, 52 torn septa were showed by 64 slices spiral CT , while 44 were showed by DSA. Among 13 cases underwent transluminal stent-graft placement , 4 leakages of type Ⅰ were found by 64 slices spiral CT. [Conclusions] 64 slices CT can show the full aorta clearly truly all perspectively and through its fast, very thin volume rendering and various imaging post-manage technology. , 64 slices spiral CT has an important clinical applicable value in all aspects , such as diagnosis of aortic dissection, accurate classfication, selection of therapy plan and the evaluation of pre - or post-operation. 64 slices spiral CT angiogrophy is a safe,non-traumatic, fast and accurate examination method.
