肾综合症出血热各级肾动脉血流动力学改变的临床意义

摘  要:[目的]观察肾综合征出血热(HFRS)病程中肾动脉血流动力学变化。[方法]用彩色多普勒超声仪检测380例经血清学证实的HFRS患者病程中各期肾主动脉、段动脉、弓形动脉的血流频谱参数与320例正常人对照。[结果]发热期、恢复期各级肾动脉血流频谱参数与正常对照组比较,均未见统计学差异(P﹥0.05)。低血压休克期、少尿期、多尿期各级肾动脉RI明显高于正常组(P﹤0.01)。Vd、Vs在低血压休克期、少尿期、多尿期低于对照组(P﹤0.05)。[结论]HFRS病程中各级肾动脉RI、Vd、Vs随病程变化有一定规律性。从少尿期到恢复期RI逐渐减低,Vd、Vs逐渐增高。检测肾动脉血流对监测HFRS病程有重要意义。[Objective]To investigate all renal hemodynamincs during hemorrhagic fever with renal syndrome(HFRS).[Methods]The blood flow spectrum of all the main renal arteries,segmental arteries and arcuate arteries among 380 patients with HFRS identified by serology were detected by using color doppleer ultrasonography,and which were compared with that of health adults.[Results]The blood flow spectrum all the main renal artery in pyrogenetic stage and covalescent period were compared with the normal control group,no significant difference were not found(P﹥0.05). The RI of all the main renal arteries in shock stage of hypopiesia,ologuria stage and diuresis stage was significantly higher than that of normal group(P﹤0.01). The Vd and Vs in shock stage of hypopiesia,ologuria stage and diuresis stage showed significant decreased when compared with control group(P﹤0.05).[Conclusion]The RI,Vd and Vs in all stage of renal arteries have prominent regularity with the course of HFRS. The RI gradually decreases from oliguria stage to convalescent period,while Vd and Vs gradually increase. To detect the blood flow spectrum of renal artery is helpful to the monitoring the course of HFRS.
