摘 要:目的探讨外阴尖锐湿疣(CA)患者宫颈人乳头瘤病毒(HPV)感染亚型状况,为临床尖锐湿疣诊断及宫颈癌的防治提供实验依据。方法采用基因芯片方法检测77例CA患者和同期无外阴CA的64例对照宫颈脱落细胞中HPV-DNA亚型(包括5种低危型和18种高危型)。结果77例CA患者标本中检出HPV阳性64例,阳性检出率为84.4%,高于对照组的18.75%(P<0.01),共检测出12种HPV基因型别,其中低危型(HPV6、11、43、44)感染21例,占32.8%;高危型(HPV161、8、31、334、5、565、8、73)感染30例,占46.9%;高低危同时感染13例,占20.3%。结论基因芯片是一种敏感性高,特异性好,适合临床HPV分型检测的方法。外阴CA患者宫颈HPV感染较为普遍,且高危型感染占有相当大的比例,因此对外阴CA患者在治疗疣体同时需进行HPV-DNA检测,必要时需进行相应治疗,以期较低宫颈癌的发病风险。Objective To study the genotypes of human papillomavirus (HPV) from the cervix in condyloma acuminata (CA)and provide laboratorial data for prevention and control of cervical cancer.Methods Gene-chips technique was used to determine 23 types of HPV genotypes in specimens of dropping cells from 77 female patients with CA and 64 controls. Results There were 64 positive cases in 77 patients with CA, The total positive rates was 84.4 %. There were 12 genotypes were detected, included the detecting total rates of 4 low risk HPV genotypes 6,11,43,44 were32.8% (21/64), while the detecting total rates of 8 high risk HPV genotypes16,18,31,33,45,56,58,73 were 46.9% (30/64),The positivity rates of mixed infection with low risk and high risk HPV was 20.3 % (13/64). Conclusion Gene chip technique is a high sensitivity and specificity method in detection and typing of HPV. Cervical HPV infection is general in patients with condyloma aeuminata (CA), furthermore the infection of high risk genotypes occupy a big proportion.The HPV-DNA should be detected during treatment of patients with CA,the risk of cervical cancer maybe decrease as expected.
