糖尿病肾病患者内皮细胞抗体表达与炎性细胞因子水平关系研究

摘　　要：目的通过检测患者血液中的内皮细胞抗体(AECA)及炎性细胞因子,观察炎性细胞因子水平与AECA表达之间的关系。方法根据间接免疫荧光法检测糖尿病肾病(DN)患者AECA的结果,将DN患者分为AECA阳性组和阴性组,并设健康组和糖尿病初诊组为对照,分别采用酶联免疫吸附法及化学发光法测定P-选择素(P-selectin)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)水平。结果健康组及糖尿病初诊组中未检出AECA,DN组检出率为19.1%;DN患者三种炎性细胞因子水平均显著高于糖尿病初诊组(P<0.01);糖尿病初诊组高于健康组(P<0.05);AECA阳性组炎性细胞因子水平也显著高于AECA阴性组,两组比较有显著差异(P<0.05)。结论炎症造成血管损伤,同时诱发机体产生自身免疫;AECA介导的免疫损伤又加重内皮细胞功能障碍,二者相互作用是DN发生的一个重要环节。Objective To discuss the relationship between the expression of serum antibodies to endothelial cells （AECA） and the levels of inflammatory cytokine in patients with diabetic nephropathy（DN） by measured them in blood. Methods According to the results of AECA,the DN patients were divided into two groups：AECA positive group and negative group;The initial onset patients with diabetes mellitus and the health peoples were as the contrast.They were all measured the levels of P-seleetin,Interleukin-6 and TNF-α by ELISA and CIA assay. Results AECA were not detected at the initial stages of diabetes mellitus and the health peoples. The positive rate of AECA in DN was 19.1% ; Three kinds of inflammatory cytokine levels in DN patients were significant above than the DM patients（ P 〈 0.01） ;The cytokine level in DM also above than the health group（ P 〈 0.05） ;Them were significant differences of cytokines levels between the positive and negative groups（ P 〈 0.05）. Conclusion The inflammation conduce the injury of blood vessel and deduced the autoimmunity. The immunity damaged mediated by AECA aggravate endothelium dysfunction. The interaction of them may .be the important role in the onset of DN.
