摘  要:目的分析福州市饮食行业从业人员肠道大肠埃希菌耐药性及其产超广谱β-内酰胺酶(ESBL)的状况。方法2006年3月-2006年6月,从福州市疾病控制中心和晋安区疾病控制中心收集716株肠道大肠埃希菌作为实验菌株,琼脂稀释法检测细菌对14种抗菌药物的敏感性,微量肉汤稀释法检测产ESBL状况。结果药敏试验显示,716株试验菌株对14种抗菌药物耐药率最高的是青霉素,达65.78%(417/716);耐药率较低的是三代头孢和三代喹诺酮类药物,均<20%。39%试验菌株对3种或3种以上抗菌药物同时耐药(多重耐药)。产ESBL菌检出率为2.51%(18/716)。除庆大霉素外,产ESBL的大肠埃希菌对其他13种抗菌药物的耐药率均高于非产酶菌(P<0.05),且皆为多重耐药。大肠埃希菌产ESBL与对庆大霉素、卡那霉素及萘啶酸耐药相关系数分别为:χ2=74.59,r=0.31;χ2=22.56,r=0.17;χ2=238.39,r=0.50。结论饮食行业从业人员肠道大肠埃希菌对14种抗菌药物耐药率在3.63%~65.78%,产ESBL大肠埃希菌的检出率为2.51%,为防止耐药株的播散,应加强监测大肠埃希菌的耐药谱。Objective To investigate antibiotic resistance and ESBL producing strains of Escherichia coli isolated from intestinal bacteria of food and drink practitioner. Methods The 716 strains Escherichia coliwas collected from OX of Fuzhou city and Jin'an districtfrom March to June in 2006. The resistance of 14 kinds of antibiotics was determined by agar medium and the detection of ESBLwas performed by broth medium. Results Among these 14 antibiotics, the highest level of antibiotic resistance was penicillin which reach to 65.78% (417/716) while the resistance to cephalosporins and fluoroquinolones were all lower than 20%. 39% of tested strains were resist to three or more antibiotics(multi-drug resistance). The prevalent rate of ESBL producing strains was 2.51% (18/716). ESBL-producing strains were performanced as multi-drug resistance and the resistance level to the other 13 antibiotics(with the exception of gentamicin)were significantly higher than no-ESBL producing strains(P〈0.05). Conclusion The drug-resistance rate of Escherichia coli isolated from intestinal bacteria of food and drink practitioner was between 3.63% and 65.78%. The prevalent rate of ESBL producing strains was 2.51%.
