帕金森病患者血清细胞炎性因子水平的变化与其相关因素研究

摘　　要：目的：检测帕金森病（PD）病人的血清免疫细胞因子的变化，探讨其变化的相关因素。方法：对51例PD病人及35例健康志愿者应用放射免疫法分别检测其血清IL-1β、IL-2、IL-6和TNF-α水平。结果：PD组血清IL-1β、IL-6和TNF-α的水平明显高于对照组（P〈0．01），IL-2无差异。4项细胞因子变化明显的是IL-6和TNF-α，与变化相关的因素是年龄、病程和服用L-多巴的剂量。高龄、长病程和服用L-多巴用量≥410mg／d者，其IL-6和TNF-α水平显著增高（P〈0．05～0．01），而性别、病情和辅加服用DA受体激动剂与细胞因子水平变化均无相关性。结论：PD病人的高龄、长病程、加之长期大量服用神经递质药物L-多巴可能是导致患者免疫功能紊乱的主要直接因素，而与PD本身无因果关系。Objective： To detect the level of immune related cytokines in the serum of the patient with Parkinson＇ s Disease （PD） and to explore the influencing factors of the cytokines. Methods： 51 patients with PD （PD group ） and 35 healthy control （ control group） were studied. The two groups were detected the serum concentration of IL-1β, IL-2, IL-6 and TNF-α by the way of radio immunity. Results： The serum level of IL-6 and TNF-α in the PD group is significantly higher than that of the control group （ P 〈 0.01）. The serum level of IL-1β in the PD group is significantly higher than that of the control group （ P 〈 0.05）. However, there is no difference in IL-2 between the two groups. The influencing factors of the difference on IL-6 and TNF-α between the two groups are age, the duration of illness and the dose of L-dopa. The old age,the long duration of illness and intake L-dopa more than 410mg/d may lead the level of IL-6 and TNF-α significantly increased（ P 〈0.05 0.01）. However,the gender,the state of illness and admit the agonist of dopa receptor can not interfere the serum level of IL-6 and TNF-α. Conclusion： The old age, the long duration of illness and the high dose of L-dopa maybe the key factors that lead to the disturbance of immunologic function in the patient with PD. It is not PD itself caused the disturbance of immunologic function directly.
