摘 要:目的观察小C臂导向下臭氧与胶原酶联合注射治疗腰椎间盘突出症的临床疗效及安全性。方法将150例腰椎间盘突出症患者随机分为A组、B组、C组各50例,3组患者均在小C臂导向下进行介入治疗。A组注入50~60μg/mL的臭氧5~15mL,间隔10min后注入胶原酶600U;B组单纯注入50~60μg/mL的臭氧5~15mL;C组注入胶原酶600U。结果①A组术后1、3、6个月的近期疗效明显优于B组和C组(P<0.01),12个月、18个月、24个月的远期疗效也明显优于B组和C组(P<0.05);②A组疼痛缓解时间(6.0±2.0)d,明显短于B组(9.5±3.5)d和C组(10.0±4.5)d(P<0.001);③A组的不良反应率仅4.0%,明显低于C组的18.0%(P<0.05)。结论臭氧与胶原酶联合应用治疗腰椎间盘突出症疗效确切,能较快缓解患者疼痛,安全性好,无并发症发生,术式操作简便易学,便于基层医院推广应用。Objective To investigate the efficacy and safety of combined injection of ozone and collagenase for the treatment of lumbar disk herniation(LDH). Methods 150 patients with LDH were randomly divided into three groups (50 cases for each). All patients were piqured in small C-arm guiding. Patients of group A were injected with 5 - 15ml of ozone (50 - 60μg/ml) ,and then with 600U of collagenase after 10 min;patients of group B with 5 - 15ml of ozone;all patients in group C with 600U of collagenase. Results ( 1 ) After the operation, all patients were followed up. The short-term and longterm effects of patients in group A were better than those in group B and group C ( P 〈 0.05 ) ; ( 2 ) The pain releasing time of group A was ( 6.0 +- 2.0 ) d, significantly shorter than that of group B [ ( 9.5±3.5 ) d ] and group C [ ( 10.0 ± 4.5 ) d]( P 〈 0.001 ) ; (3) The adverse reaction rate of group A was 4.0% , significantly lower than that of group C ( 18.0% ) ( P 〈 0.05 ). Conehtsion Combined injection of ozone and eollagenase for the treatment of LDH is of certain effect, easy operation, safety. no complications, worth popularizing.
