摘 要:目的:从临床诊断要求、患者辐射剂量及所需成像技术条件等三方面,探讨建立胸部照片综合评价的方法和标准。方法:取10份正常胸片,并参考文献,确认胸部正位照片评价区域的划分及诊断要求的标准;通过男女各20例胸部CT扫描,计算出肺野与纵隔部比例;取125 kVp条件下摄影胸片100例,测量各评价区的标准密度值;测量男女共478例,正常成人体表入射剂量。结果:从诊断要求出发确认7个评价点,物理因素确认为4个评价因素,并给出了评价标准;建立综合评价方法;初步探索了中国正常成人标准身高、体重、体表入射剂量。结论:从综合评价概念出发,就胸部正位影像评价方法和标准做了探讨,为今后建立全国正式标准提供了方法和思路。Objective PurposeBased on clinical diagnostic demand, patient irradiation dose and imaging technical paramenters, to establish a comprehensive evaluation method and standard in chest radiograph. -Methods From 10 normal chest radiographs, we selected the evaluation areas on thoracic PA (posteroanterior) radiographs and set up standard for diagnostic de- mand; Using chest CT scans of 20 males and 20 females, we calculated the ratio of lung field to mediastium; Selecting 100 chest films using 125 kVp, we measured the standard density values of each evaluation area; Body surface irradiation doses of 478 normal adults were measured. Results Based on diagnostic demand, we confirmed 7 evaluation areas and 4 physical evaluation factors. At the same time, evaluation standards were obtained ; Comprehensive evaluation methods were established ; Standard height, weight and body surface irradiation dose of Chinese normal adults were investigated preliminarily. Conclusion Based on the concept of comprehensive evaluation, investigation on the evaluation methods and standard in chest PA radiograph was carried out which might be taken as the foundation for future approach on nation-wide basis.
