2002～2006年龙岩市县级以上医院肺结核病人转诊情况调查

摘　　要：[目的]了解龙岩市县级以上综合医院肺结核病人的转诊情况,为促进结核病归口管理工作提供依据。[方法]抽查医院相关科室的结核病登记和转诊资料,将医院抽查到的肺结核病人与结防机构进行核对,计算转诊率和转诊到位率。[方法]2002～2006年共抽查医院诊断的肺结核病人916例,已转诊782例,转诊率85.4%,转诊到位553例,转诊到位率70.7%。各年转诊率分别为92.7%、90.5%、83.4%、93.9%、68.5%,各年间转诊率差异有统计学意义(P<0.01)。各年转诊到位率分别为71.6%、75.4%、71.2%、64.9%、67.6%,各年间转诊到位率差异无统计学意义(P>0.05)。县、市级医院转诊率分别为93.9%、61.1%,差异有统计学意义(P<0.01)。县、市级医院转诊到位率分别为73.6%、58.2%,差异有统计学意义(P<0.01)。[结果]县级以上综合医院(尤其是市级医院)的转诊率和转诊到位率总体偏低。加强与综合医院合作,建立完善的转诊制度,是落实结核病归口管理的重要途径。[Objective]To understand the status of TB referrals in general hospitals above county level,provide scientific basis for the management. [Methods]Records and referral data in related departments were selectively examined . Data of TB patients selectively examined in the hospitals were checked ycith the data from TB Control and Prevention Institute . Referral rate and visiting rate were calculated. [Results]916 TB patients diagnosed in hospitals were selectively examined from 2002 to 2006. There were 782 referrals, accounting for 85.4%. There were 553 visiting patients, accounting for 70. 7%. The rate of referral was 92.7% ,90. 5% ,83.4% ,93.9% and 68.5% respectively from 2002 to 2006. There was adifference （P〈0.01）. The visiting rate was 71.6% ,75.4% ,71.2%,64. 9% and 67.6% respectively fror0 2002 to 2006. There was no difference（ P〉0. 05）. The rate of referral was 93.9% and 61.1% in hospitals at county and municipal level. There was a difference （ P〈0.01）. The visiting rate was 73.6% and 58. 2% in hospitals at county and municipal level. There was a difference（ P〈0.01）. [Conclusion]The rate of referral and visiting was low in hospitals above county level,the key of TB management was to strength the cooperation with general hospitals, set up completed referral system.
