用超声心动图测量陈旧性心肌梗塞乳头肌收缩功能

摘　　要：应用超声心动图测量正常人及陈旧性心肌梗塞患者左室乳头肌长度及功能评价陈旧性心肌梗塞患者乳头肌功能紊乱与二尖瓣返流间关系。结果表明：正常人前外侧后内侧乳头肌短缩率及后内侧乳头肌收缩末长度与陈旧性心肌梗塞患者比较存在差异（Ｐ值＜０．０１，Ｐ值＜０．０５）。陈旧性心肌梗塞合并中、重度二尖瓣返流患者前外侧后内侧乳头肌短缩率（ＦＳ）及后内侧乳头肌收缩末长度（ＬＳ）与正常组比较存在差异（Ｐ值＜０．０１），而轻度二尖瓣返流组只有后内侧乳头肌ＦＳ与正常组比较存在差异（Ｐ值＜０．０１）。The purpose of this study was to assess the length and contractile performance of human left ventricular papillary muscles and to dtermine the relationship between papillary muscle dysfunction and mitral ragurgitation in patient with old myocardial infarction.The results showed that there was a significant difference in the end-systolic lengths of the posterior papillary muscle and fractional shortening of both the anterior and posterior papillary muscles between the normal and old myocardial infaction groups(P<0.01,P<0.05,respectively).The fractional shortening in the group with moderate to severe regurgitation was significantly decreased than that in the normal control subject for both the anterior and posterior papillary muscle(P<0.01).However,Fractional shortening in the group with mild regurgitation was significantly reduced than that in the normal control subjects for the pasterior papillary muscle(P<0.01).Therefore echocardiogrphy is of high climical value for assessing the abnormal contractility of human left vrntricular papillary muscles.
