行为干预在小儿出口梗阻型便秘治疗中的临床作用

摘　　要：目的探讨行为干预措施在小儿功能性出口梗阻型便秘治疗中对临床症状改善的作用。方法对38例确诊为出口梗阻型便秘的患者进行便秘症状评分,并随机分为干预组与对照组,对照组给予一般药物治疗,干预组在对照组的基础上给予合理饮食、适量饮水、调整生活习惯、适当运动、精神减负、腹部按摩及排便训练等一系列行为干预措施辅助治疗,治疗后两组病例再次进行便秘症状评分,并比较评分结果。结果干预组治疗后的直肠初始感觉值与最大耐受值明显低于本组治疗前及对照组(P<0.01,P<0.05),干预组与对照组各组的治疗后便秘临床评分均低于治疗前,差异有统计学意义(P<0.01);在两组的组间比较中,治疗前评分差异无统计学意义(P>0.05),治疗后干预组的便秘临床评分明显低于对照组(P<0.01)。结论通过行为指导与训练配合药物治疗能明显减轻出口梗阻型便秘患儿的临床症状,提高疗效,达到单纯药物所不能替代的作用。Objective To study the symptom relief function of behavior intervention methods on pediatric functional outlet obstruction constipation treatment. Methods Constipation symptom scores were made in 38 confirmed outlet obstruction constipation cases. Then they were randomly divided into intervention or control group. The control group received general medication and the intervention group received a series of behavior methods such as reasonable diet. Appropriate water intake, living habit change, appropriate exercise, psycological alleviation, stomach massage and howe hraining as well as general medication. Constipation symptom scores were made again after these treatment and comparedwith the former scores. Results Post - treatment constipation symptom scores were lower than pretreatment scores in both groups and there was obvious difference（ P 〈 0.01 ）. There was no obvious difference between pretreatment constipation symptom scores in both groups（ P 〉 0.05 ）, Post -treatment constipation symptom scores in intervention group were much lower than in control group （ P 〈 0.01 ）. Conclusion Behavior guide and training combined with medication can obviously relief symptoms of outlet obstruction constipation patients and improve treatment effect. It has irreplaceable function which can not be reached by simple medication.
